Forest Creek Village is a senior living community located in Indianapolis, Indiana, offering a full continuum of care that includes short-term rehabilitation, memory care, long-term care, respite care, hospice care, and outpatient therapy. The community emphasizes a person-centered approach, aiming to become an extension of each resident's family while helping individuals and their loved ones find answers, solutions, and peace of mind.
Short-Term Rehabilitation is designed for those recovering from stroke, cardiovascular difficulties, orthopedic surgery, and other debilitating conditions, with the goal of helping participants return home safely. The program includes physical, occupational, and speech/language therapies supported by an interdisciplinary team of doctors, nurses, therapists, and social workers. Through the Road to Recovery program, the care team meets with patients and families within the first 72 hours of admission to map out discharge goals. Additional services include wound care, fall prevention, contracture management, mobility training, and strength-building wellness programs.
The memory care program is structured and research-based, using a person-centered, wellness-based model that incorporates stimulating daily activities to sustain independence and promote a healthy spirit. The approach focuses on maintaining a secure environment where residents experience a sense of belonging and meaning, supported by a Meaningful Day activity program built on four domains of wellness.
Long-Term Care is available for those who can no longer manage daily tasks at home due to illness, injury, frailty, or other limitations. Care is provided under the guidance of an attending physician by licensed nurses, trained health care aides, and an interdisciplinary care team, and includes social services, activities, and therapy services in home-like, secure settings. The community accepts private pay and a variety of payment options, including Medicaid for qualified applicants, with specialists available to assist families through the application process.
Respite care offers short-term support for family caregivers who need a break to travel, recover from illness, or attend to other responsibilities, with the flexibility to extend care if needed. Hospice and palliative care services are provided in partnership with local hospice companies, offering skilled nursing, spiritual and bereavement counseling, medical equipment, and social services through an interdisciplinary team. Outpatient Therapy rounds out the offerings, combining trained therapists and current technologies to support mobility, reduce pain and inflammation, and promote overall health in coordination with each resident's physician.
