I've never stayed at such a run down nursing home. The nurse Brittany was so so rude and they did nothing about it. It smells like urine so bad because there housekeeping is terrible. I'll never stay at a ASC community ever again in my lifetime. I'd rather be homeless. This place doesn't even deserve a rating. I don't know how the state has not shut them down. Thank you for listening. Have a nice day.

My name tonya Anderson I had the pleasure of staying at rehab nursing and facility on 16th St. I was very pleased with my staff that I had Jordan. The nurse was excellent to me and Britney and so was the nurses in QMA‘s on the second level they were all super nice they even a young lady they work in activity room and the person that came around and took my meal me you everyone was super nice to me there and they also the lady that clean my room I’m named Sue. She was super nice. I wouldn’t recommend anybody to go back to nursing and rehab facility hall E. 16th St.

So I see all these reviews on here about how good this place is well let me tell you some truth<br>They are not as family friendly as they say<br>So they leave there patients in bed in there own feces all day and even if the patient or patients family call for them they either don’t come or say they will and never show and if you say something they say oh we forgot and they don’t clean the patient and they whole place smells so bad most of the time like urine and they never clean the rooms so the room is always nasty<br>And you will find medication all over behind the bed and under it and sometimes in the middle of the floor because there not watching the patients take there meds so they just throw them on floor or drop them and they don’t pick<br>It up because they didn’t see it happening because they weren’t watching like they where supposed to and they don’t keep track of the meds in the computer they just give them what was on the hospital list when they got there atleast that’s what they told me when they where giving my dad the same medication in 2 different milligrams 3 times a day and he was hallucinating and had him down as a diabetic and mad him take shots for something he didn’t have wrong with him and if that ain’t bad the way they talk to there patients and treat them is awful and it a bunch of young kids usually running the place I mean the nurses .<br>And on top of all that you will get your money and cloths and belongings stolen by who I don’t know I never could tell but if your wondering I had him moved somewhere else<br>But I do not recommend puttIng anyone in here<br>Oh and they just hand the codes to the door out to anyone so someone could walk in and out with you family members and no one would know so again no I don’t recommend this place at all it’s nasty and wrong
