About Beech Grove Meadows

Beech Grove Meadows is a senior living community in Beech Grove, Indiana, offering short-term rehabilitation, memory care, long-term care, respite care, hospice care, and outpatient therapy services.

Short-term rehabilitation is designed for individuals recovering from stroke, cardiovascular difficulties, orthopedic surgery, and other debilitating conditions. The program includes physical, occupational, and speech-language therapies along with support from doctors, nurses, therapists, and social workers. A hallmark feature is the Road to Recovery program, which brings together the interdisciplinary team, patient, and family within the first 72 hours of admission to establish discharge goals. Services encompass wound care, fall prevention, mobility training, strength-building, and aerobic conditioning programs.

The memory care program serves individuals with Alzheimer's disease and other forms of memory loss through a structured, research-based approach. Using a person-centered, wellness-based model, the program incorporates stimulating daily activities designed to sustain independence and promote well-being while maintaining a secure environment where residents feel a sense of belonging.

Long-term care is provided for residents who struggle with daily tasks and can no longer remain at home. Licensed nurses, specially-trained health care aides, and an interdisciplinary team deliver care under physician guidance, including social services, activities, and physical, occupational, and speech therapies to help residents maintain independence. The community accepts private pay and Medicaid for qualified applicants.

Respite care services offer temporary stays for those needing short-term support, allowing family caregivers to travel, manage new employment, recover from illness, or pursue other needs. Hospice and palliative care provide comfort, pain management, and emotional support for those facing terminal illness through an interdisciplinary team approach. Outpatient therapy uses clinically-proven strategies and modern technologies to promote mobility and reduce pain under physician coordination.

I am writing this review on behalf of my mother, Malinda Smith but in her words. I was sent to BGM after falling and needing rehab. From the first day till my last day the food was very very good I always ate every bite, and was made according to what I could eat. Thank you kitchen staff each and everyone special thanks to Bonnie, Mike and Sabrina.I arrived on June 7th 2026. Jason and Cathy made sure and sometimes Cory that my room was very nice and clean, I never had to complain . I had the best nurses looking after me, Kathy, Wendy, Olivia, Alexus, Tay Tay,Nykittra, Amy,these ladies were amazing and took great care of me and I know that they cared about their residents not just a check. Thank you to the two nurses Clarissa and Vanessa who took care of my wounds. The aides were very good at taking care of me to name a few, Nia, KeKe, Jay, they always treated me with a smile and ask me how I was feeling . I have to say Nia gave me the best showers, I would sleep so good! If I forgot anyone please know it’s because there were so many wonderful staff members. I wànt to thank Patti in activities for being so patient and kind. I say all this to let people know that reads this how wonderful my stay at BGM was I was there for around a month. It was the best place to be and if I had to go back into long term I want to come back to BGM. The place was beautiful, clean and always smelled good. Thank you Beech Grove for a very special experience. In my words as Malinda daughter I would like to say that sometimes we as family may not like something or feel a way about something and explode but when your love one loves the place and gives it 5 stars then listen to them because that is who it is about . Don’t push your negativity feelings on them. My mother always said she never wanted to be put in a nursing home but when she got to BMG she said ( I am so glad I chose this place) Thank you BGM for getting my mother to trust and feel safe, you guys ROCK!!

My mom was sent to Beech Grove Meadows earlier this year for rehabilitation after a hospital stay, and later returned following a second hospitalization. I was initially worried because of some of the reviews I had read; however, the staff at Beech Grove Meadows exceeded my expectations. In fact, we had recently decided to keep her there for long-term care. Unfortunately, my mom eventually passed away due to her illness, and I am incredibly grateful for the kindness, dignity, patience, and compassion the staff at Beech Grove Meadows showed both my mom and me throughout her time there. Since I live out of state, it was impossible for me to be with her every day. Still, I remained in constant contact with the facility and always felt confident that she was receiving the best care possible. I want to sincerely thank everyone who cared for her, checked on her, comforted her, and supported us along the way. During the last week of her life, so many staff members took the time to visit and care for her, even when they were not assigned to her. That meant more to me than I can express. I will forever be thankful for the love and care my mom received at Beech Grove Meadows.

My son has been a resident for a little over a year and I can not give this facility enough praise for the excellent care he has received! As a young quadriplegic, who is also autistic, it’s been difficult to find a place that is accepting and feels like home to him. He has been in 5 facilities in the 7 years since his accident and while none will be perfect, Beech Grove Meadows certainly comes close! The staff treats him like their own which makes this momma’s heart very happy!

We originally chose Beech Grove Meadows for several reasons, but four stood out most: the attentive staff, the dining facilities, the step-up care options, and the ability to customize my father’s unit. We even made a substantial investment in improvements, including a high-quality fence and a dog door installation, to make this feel like home. Unfortunately, much of what made this community appealing is no longer available. The dining room and assisted living care have been discontinued, yet rent was raised substantially—an increase that feels unjustified given the reduced services. My father went several days without air-conditioning, and nothing was done until I threatened to purchase a portable unit myself. I have left multiple messages for Administrator Jeremiah Johnson and have never received the courtesy of a return call. The staff members themselves remain wonderful, but they are clearly limited by a management team that does not support them. Only 15 months later following the difficult move from our family home of 46 years, we are now faced with the painful decision of relocating my father once again. Based on our experience, I cannot recommend Beech Grove Meadows to anyone looking for quality care, support, and open communication for their loved ones.
