About Elevate Care Des Plaines

Elevate Care Des Plaines is a skilled nursing and long-term care facility in Des Plaines, Illinois, with a licensed capacity of 275 residents. The community holds a 3.9/5 rating across 47 Google reviews, reflecting significant variation in family experiences.

Families consistently report problems with communication among medical staff and between staff and families. Some described being given reassuring information during tours that did not match their experience after admission, and staff unable to provide clear answers about a loved one's medical status or treatment progress. Cleanliness and facility maintenance concerns appear repeatedly in lower-rated reviews, with families noting old furniture, stained bedding, and bathroom odor issues. One family wrote: "The cleanliness leaves a lot to be desired." However, families who rated the facility highly often praised specific staff members, particularly the activities team, for bringing genuine compassion and engagement to daily life. A few families also highlighted helpful staff support during recovery planning and regular communication about care and activities.

The gap between positive and critical reviews suggests inconsistent experiences, possibly depending on which department or staff member a family engages with. Prospective residents and families should plan to ask specific questions about medical communication protocols and facility maintenance during visits.

Recent story: Mom is 95 years young. And, up until 2024, she was still driving, very independent, and, often on her own to shop or visit friends over lunch.

In 2024 mom was involved in a minor traffic accident. Not her fault. But, one consequence of the accident was a reduced confidence in driving.

Family has helped with errands and shopping. But, she still longs for that quiet time, to visit with her friends.

More recently, in April of this year, Mom had an accident at home. She ended up in the ER at a local hospital. She was admitted for five days of observation to intensive care.

On May 5th she was released from “hospital care” and transferred to a rehab facility, near her home. Under a new Corporate structure, the site is now known as Elevate Care.

Every week-day she has therapy to rebuild her strength and her confidence. Elevate Care is a tremendous organization for patients in need of daily therapy & some necessary medication for pain management.

Elevate Care, formerly “Oakton Arms”, offers daily therapy, nursing and physical support. And, offers a caring staff to guide her recovery.

I am happy to review & compliment Elevate Care for treating mom with compassion, understanding and persistence.

Hoping that mom will regain some of her functioning capacity and show off that resilient spirit that we know is alive inside her.

Thank you to the tremendous staff at Elevate Care.

The employee are just wonderful and very helpful and kind!! The new company that just acquired Oakton Pavilion Wont leave any supplies in the room as needed. So i had to get nonprescription items like Dakin's cleaner for wounds to keep in the room,
Also not a prescription zink oxide past!!! NOT ALLOWED to have ready in the room either!!!! Very sad, we just want the best for our loved ones.
Also the new company took out our loveseats from the room!!! Replaced with uncomfortable chairs. You would think the company would want the patience family to think of the place as home.
Very sad that we have to be put through this at our worst time ever.
It used to be a happy patient is a happy life.
Enough said!!!! they'll probably try to kick me out now.
Thank you for reading. There's my 2 sense worth!!!!
