Well in general this an excellent. Nursing home with a few bad Cnas one bad CNA is named ArlinHe has good skills but a poor attitude he’s lazy and because he’s unsupervised very slow in my opinion I’ve been tursing homes in my rehab journey and Elevate Northbrook has very good rehab. The PT named Tiju is excellent at her job. I’ve reach my records personal best walking distance. Over 48 feet with her as my PT. They have very good occupational therapy with Shannon and Caroline. As Excellent OTs. The food is not terrible. Tal is good kitchen manager. Mary is an excellent administrator she’s present and attentive in a demanding role obviously she’s experienced and she runs a pretty tight ship here at Elevate Northbrook. Another very good CNA is Natalie she’s attentive has excellent skills and a great attitude. She takes her time to be sure she does everything required for her residents. I’d highly recommende at his role. They’ll heat up food that’s not real hot. Alice is very skilled with generally a pleasant attitude CNA isn’t an easy job so occasional times you’ll be having to wait for your turn to be taken care of but I measure skill and attitude as I did in my over 30 years in management. And the same still goes in my nursing home journey. In general except for a few questionable cnas. Elevate Northbrook is a 6/10 in my view

My loved one was on the Medicaid floor (third floor) for several months, not by choice. I want to be clear that people on the private pay floors may have a different experience, but the building and management issues affect everyone. The facility appears to be a converted apartment building and it shows — peeling paint, overcrowded conditions, and a wheelchair ramp that is a switchback spanning an entire story. I have no idea how this passes inspection. Basic things that were broken — his TV remote, his adjustable bed — were never fixed despite repeated requests. No phone in the rooms. Physical therapy was promised over and over and almost never happened, even though his insurance was covering it. The dining room on the Medicaid floor has a loud TV and the second floor dining room has two giant TVs blaring on different channels simultaneously. Who thinks this is appropriate for people with cognitive issues? There is no outdoor space residents can access on their own — they have to be signed out to sit on a bench next to a busy road. There is no real enrichment. The salon was closed the entire time he was there despite being advertised. There are no rooms for people to meet in even with their doctor. One of the doctors conducted a private medical evaluation in his room with his roommate present and the door open. The good: some of the nurses and CNAs are genuinely wonderful people and I don't know why they stay. One nurse's knowledge got my loved one to the hospital quickly for a developing infection. Another CNA took it upon himself to cut my loved ones' hair every couple of weeks since the salon was closed. The office manager was highly responsive and pleasant to work with. The overall management of this facility is the problem as well as the facility itself which was not designed to be a nursing facility. My loved one is now in a beautiful modern facility and even the lesser facilities in our new state are better than Elevate Northbrook.

If I could put zero stars I would. My family’s experience with Elevate Care Northbrook was beyond unacceptable, it was dangerous. My grandfather stayed there for not even a month and required close overnight monitoring. Instead, he was neglected. On multiple nights, staff failed to properly supervise him, and his condition deteriorated to the point where his oxygen levels became dangerously low. Something that should never happen under any competent standard of care. This was not a simple oversight; it was a serious failure in basic medical responsibility. A facility entrusted with vulnerable patients should never allow this level of neglect. Families place their loved ones’ lives in the hands of facilities like this. In our case, that trust was completely broken. I strongly urge others: DO NOT admit your loved ones here. If you have any alternative, take it.

My wife has been at the facility for a couple years. I visit her for a couple hours every single day. There are a lot of wonderful people who work there, they do the best they can with what little they have. Since I'm there every day I see patterns that others might not see. Regarding that there are some basic things that I do for her every day. My goal is to make sure she is clean and as comfortable as possible. I love my wife and I will do absolutely anything for her. I've learned that certain people will always try to pass blame on me but it is what it is. In the end I know the truth. A side note Michelle and Brooke are the best cna's on the planet, there are real angels who walk the earth. They are two of them.
