Our family is so grateful for the care my grandmother has received at Elevate Care over the past six months. It is never easy trusting others with someone you love, but from the very beginning, the staff has shown her so much kindness, compassion, and patience. They have made such a positive difference not only in my grandmother's life, but in our entire family's as well. I especially want to recognize my grandmother's nurse, Eman, and her CNA, Didi. They are truly exceptional. The patience they show my grandmother means more to our family than they probably realize. They never rush her, they always take the time to listen, and they consistently treat her with kindness, dignity, and respect. You can tell they genuinely care about the people they look after, and that kind of compassion is something you simply can't teach. Knowing my grandmother has Eman and Didi caring for her gives our family so much comfort and peace of mind. Didi is truly an angel in our eyes. She has such a warm, fun, and caring personality, and she always makes my grandmother feel comfortable and valued. Every interaction we've seen has been filled with kindness, patience, and genuine compassion. She has such a positive energy, and it is clear she loves what she does. We are also so thankful for Brandon in transportation. He always makes sure my grandmother's transportation to her doctor's appointments goes as smoothly as possible. He is dependable, organized, and always willing to help. It's one of those behind-the-scenes roles that often goes unnoticed, but Brandon truly makes a difference, and our family appreciates everything he does. A huge thank you as well to Elizabeth and Kennedy in the administration. They have been incredibly accommodating, responsive, and understanding whenever we've needed anything. They have always taken the time to answer our questions and help us navigate everything with kindness and professionalism. Another thing that has made such a difference is the activities program. The variety of daily activities has helped bring my grandmother's spirit back. She has opportunities to socialize, stay engaged, and enjoy herself, and it's been wonderful to see her smile more and genuinely look forward to participating. The staff does an amazing job creating a welcoming environment where residents can feel connected and involved. Thank you to everyone at Elevate Care for everything you do each day. We know this work is not easy, and we are so appreciative of the compassion, patience, and dedication you've shown my grandmother. Eman, Didi, Brandon, Elizabeth, Kennedy, and the entire team have made what could have been a very difficult experience so much easier for our family. We are truly grateful for all that you do. Thank you for making Elevate Care feel like more than just a nursing home, it truly feels like a place where the residents are cared for with compassion and dignity.

I cannot recommend Elevate Care based on my father’s experience. My father was admitted following a recent leg amputation while also recovering from sepsis and requiring dialysis. He entered the facility medically fragile and dependent on staff for safe transfers and specialized care. Unfortunately, we witnessed repeated failures that put his health and safety at risk. The most serious incident occurred during a transfer when staff failed to follow his non-weight-bearing restrictions and placed weight directly on his newly amputated limb. The transfer was performed improperly, causing significant bleeding from his surgical site and resulting in additional medical complications that ultimately required him to be sent back to the hospital. During that same transfer, staff also failed to properly secure his Foley catheter. He ended up standing on the catheter, causing trauma that later resulted in an infection and required an additional medical procedure. These injuries were preventable with proper training and attention to basic patient care. Throughout his stay, it became apparent that many staff members lacked the knowledge and training necessary to care for patients with complex medical needs such as recent amputations, Foley catheters, and dialysis. Communication between staff members was inconsistent, and there appeared to be a lack of accountability regarding patient safety. Another serious concern involved his bed. My father, who had a recent amputation, was placed in a bed that was too short, forcing his amputated limb against the footboard. We reported the issue on Friday and again on Saturday, but staff only placed pillows at the end of the bed rather than correcting the problem. On Sunday, the nursing supervisor agreed that the bed needed to be extended but stated that only maintenance could perform the adjustment and they would not return until Monday. As a result, my father remained in an improperly sized bed for approximately three days despite staff acknowledging the issue. No patient recovering from a recent amputation should have to endure that. These experiences left our family questioning whether the facility was adequately staffed and whether employees had received appropriate training for patients requiring specialized care. We expected safe transfers, proper catheter management, attention to physician orders, and prompt responses to safety concerns. Instead, we experienced preventable injuries, delayed corrective action, and unnecessary suffering. Families place tremendous trust in skilled nursing facilities during some of the most difficult times of their lives. Based on our experience, I would strongly encourage others to carefully evaluate whether this facility is equipped to safely care for loved ones with complex medical needs before choosing Elevate Care.
