Moving Forward Rehabilitation
Moving Forward rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.
Our person-centered program is tailored to the individual. Moving Forward includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.
Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.
Returning home as quickly as possible is always the desire. Great place to go if you need physical therapy, you feel like family in here. As the resident council president, seeing the changes that need to be made get done after our meetings is amazing. They understand our needs and respond amazingly.

This facility is an absolute joke. My family member was there for 2 weeks and received thee worst care. Her bedding was changed max 2x a week (found fecal matter on her sheets and where they just covered it up with a soaker pad), wounds were not taken care of properly even when instructed by the wound doctor, nurses do not communicate with each other or with us on anything, morning meds were always late and given after lunch, caught several nurses in lies (said they gave meds when they actually didn’t , said they cleaned wounds when actually didn’t) and they tried to say my family member must have been confused, bathroom was never cleaned, floors were swept once the entire time, the food is absolutely atrocious and not edible they either need a new head cook or need to make new menus. The whole purpose of my family member going there was for rehab to be able to get her independence back, she would have been better off at home with either in home therapy or someone taking her to a therapy office a few times a week. I have worked at a nursing and rehab facility and have had other family members in other facilities , and this is by far the worst.
