Moving Forward Rehabilitation

Moving Forward rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.

Our person-centered program is tailored to the individual. Moving Forward includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.

Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.

Returning home as quickly as possible is always the desire. After previous challenges with other rehab facilities, our family was cautious about my father’s placement. We are so grateful for the exceptional experience we had here. The team demonstrated a deep understanding of his medical and personal needs, prioritizing his comfort at every turn. It was a relief to see him treated with such genuine compassion and professionalism. We very much appreciate every staff member that guided us through this process.

After witnessing my grandfather’s stay at Heritage Park, I can confidently say this is not a facility I would ever recommend to anyone considering care for their family. Over time, I began documenting multiple concerns throughout his room and other areas of the building. These issues were not isolated incidents — they appear to be ongoing and widespread. In one instance, the wall directly behind my grandfather’s bed was severely damaged and crumbling, left in that condition for an extended period of time. It became clear that these repairs are often ignored until a new resident moves in, rather than being addressed promptly for the safety and dignity of current residents. In one of the photos…the call light and electrical unit were completely pulled out of the wall, with exposed cords and unsecured wiring. This is deeply concerning, as call lights are essential for resident safety and emergency response. Leaving equipment in this condition poses a clear safety hazard. There was also a time when I personally noticed what appeared to be black mold inside an air-conditioning vent. I immediately reported this to staff, who stated they would inform maintenance. According to my grandmother, she was later told it was “nothing to worry about.” To my knowledge, nothing was ever done. Cleanliness is another major issue. My grandmother’s room frequently goes days without proper cleaning. Floors, tables, and surfaces remain cluttered, and on multiple occasions I have walked in to find feces on the floor or near the bed. This is unacceptable in any care setting, let alone one housing elderly residents who rely on staff for hygiene and sanitation. What makes this even more troubling is that staff members often appear disengaged or annoyed when concerns are brought up. Despite seeing housekeeping staff present in the building, rooms are often left dirty and neglected. Overall, this facility shows a lack of accountability, maintenance, and basic resident care.

Horrible place to bring a loved one who needs rehabilitation. If I could rate it 0 stars, then I would. The staff are extremely overworked… and the residents are left abandoned. Big lack of communication between staff which puts residents in harms way. I would not recommend, PLEASE don’t send your family here. Unsafe living conditions, the maintenance have no sense of urgency. Just don’t think the quality is within expectations with what the costs are to live there.

hi first thing i want to inturduce myself jimmie harrington there is a lot of staff members here that dose nt get the well desered recongniton there are a fue of the staff members that hav made life here very much easier i have had opportunity to see just how these people here are the residentshere are being treated well and the care givers here are doing very awesome job . i belive that there are many residents here that would agree with me that some of the aids here need to be recongnize for all there hard work in taking time to make the resedents comfortable the nursing staff mostly are doing a very good job there is just one of the nuring staff member that i belive that need to up his services there is five aids that work very hardi can name them in time i would to know if their is any way for H P to tunistic recongnize the importan staff that is making a big difference thank you for allowing me give my opinion i have been here over two months now andthe health care i have been getting is very good i just wished i could show this staff more appreciation i tell them thank you all the time if the management would acknowledge them would be a great thing for this staff ok now please take and thank you jimmie harrington
