Do not send your loved ones here! If you care about them at all do not put them here, especially if they can not communicate for themselves. This place is a literal nightmare! My mom sadly passed away while she was staying here. My sister and I went through so much trying to get these people to actually care for my mom. My mom constantly fell at this place , they blamed her and not their staff. She ended up having a bump on her head and lost a toenail , she said a male worker was in the room at the time but the manager just kept saying that my mom was not in her right mind and wouldn’t acknowledge that there was someone there who caused her to fall. They magically lost her phone for days after this incident happened and then about a week later found it and said they made a mistake and washed it in with her sheets! The SIM card was missing! They took her phone to hide something! I’m not sure what. This place never cleans the rooms and they barely clean the bathrooms! I paid to get my mom’s hair washed and told her it would be washed the next day. They never washed it when I came the following day or the next or the next. They kept saying , oh the person who does it isn’t here. After 2 weeks I finally just ended up asking for my money back. This place never answers their call lights and they constantly had nasty attitudes when you asked for help with your loved one, no matter how small the ask is! I wouldn’t put a Dog in this facility, let alone another human being! There is so much more that my mom went through at this place but it’s too much to list! DON’T send your family here!

This post is to recognize a continuous job well done by Christina, unit coordinator for LTC, 2nd floor. In addition to the medical appointment/transportation efforts, I've gone to Christina regarding other issues, and she has either resolved the issue or directs me appropriately. It is extremely helpful to have a knowledgeable point of contact for family members, as all teams are incredibly busy and focused on the care of the residents. We greatly appreciate the support.

My brother was in Autumn Lake Parkville for Rehab. Every time I came to visit, there was no one who was friendly at the welcome desk. They didn’t even greet you as you came in. Secondly, when you call for assistance, they were just ignore you and it would absolutely have to take a family member to make a complaint before they take action or to provide attention to you. My brother’s rehab was only for 10 minutes Which they call walking from his room to the room’s door. The food was absolutely horrible, and most of all they have roaches, crawling all up and down the wall. The staff the nurses no one there is friendly, and this place should absolutely be cited by the state for poor conditions, and staff needs to be retrained. I wouldn’t even give this place a one star

My occupational and physical therapy were very encouraging. My physical therapist pushed me to do things I didn't believe I could do and I really appreciate them for that!. I started improving in areas I didn't believe I would. The two of them were the most pleasant people to work with. Thanks you team!

My wife has been "stuck" in this facility for about three weeks now. Our experience has not been good. They have small rooms and cram three people in them with only one bathroom! They can do better with housekeeping as it smells a little worse than the typical nursing home. The communication level between the staff, patient and their loved ones is very poor. Myself and my wife who is the patient there are constantly being given the run-around in regards to her discharge, i guess they want to keep her there so they can keep billing the insurance company, They dont provide a tv in the rooms once you go upstairs to the "long term" care unit and they will not return your phone calls, no matter how many messages you leave. The social worker for this facility "Tonya" is never available and also does not return calls. It feels more like a prison than a nursing home as nobody is ever "able" to give you the information that is needed. They are also not managing her pain as well as she needs them to. Some of the staff's beside manners are not the best, they walk into a room and don't acknowledge the visitor. They had me waiting to speak with a Dr. that never showed up and they seem to be always late when setting up "care meetings" and "trainings". Its sad because our first experience with them was great; although it was the Autumn Lake on Perring Pkwy and Oakleigh and not this one. The only reason they don't get one star is because they do have a handful of people who seem to actually do their job well. Rest assured once they finally decided to discharge her, She wont be coming back to this one. It is really a shame to have this kind of an experience with people representing the healthcare field because you are only there because you actually need them! Very disappointed!
