This is the worse pace from housekeeping all the way up to the administration. The social worker Brook she is the worse social worker I ever met. The temp administrator Rob was useless. The aides are not much better. They don’t take care of the residence at all. My son’s father wa in for a short period of time. A lot of his stuff was stolen and not returned and the did nothing about it. It was awful and should be shut down. When he got discharged the gave me a bag of med and didn’t tell me anything about them. When he takes them again or nothing. Brooks was supposed to send out Referrals for occupational therapy, physical therapy and speech therapy and she didn’t do any of it. Beware if you have to go there.

Pt is the only good thing here. Food comes luke warm. Patients wondering into my room that should be supervised 24/7 . Give a monthly schedule two weeks ago of thing to do and have no idea where to go. Ring for assistance don’t hold your breath. I rate this place as minus 2 Stars!!!!!

I spent 7 weeks here in short term rehab and was impressed by many star employees, including the APRN Christina, psychologist Lewis, nurse Cici and amazing therapy team, including OT Danielle and manager of rehab therapy Carol Ann. Thank you for everyone who supported me in my goal to return home!

Horrible. Social worker brooke discharged my dad after repeated attempts and multiple medicals stated hes unsafe to take care of himself. She from day one did not want my dad there. So after helping him end his voluntary conversership, she released him the next day it ended. And within 4 days my dad was being admitted as a trauma patient due to his unsafe behaviors since again hes unable to care for himself. Please dont send your loved ones there.

My mom spent just about a month with us here at Apple Rehab in Middletown. What a nice experience. From the moment she arrived she was cared for and looked after. She can be a little difficult due to her dementia behaviors, but the staff took it all in stride. From the nurses, CNAs, recreation staff, and even the administrative team, she was always kept safe, occupied with appropriate activities, and she was always dressed and clean. Thank you everyone for a great experience!
