Moving Forward Rehabilitation

Moving Forward rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.

Our person-centered program is tailored to the individual. Moving Forward includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.

Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.

Returning home as quickly as possible is always the desire. However, returning home without the proper rehabilitation can greatly increase the chance of seniors experiencing a setback resulting in rehospitalization. Prior to returning home, our team, in partnership with the patient and family, will conduct a comprehensive in-home safety evaluation to ensure patients can successfully navigate their home with confidence.

Physical Therapy focuses primarily on independent mobility, regaining strength and balance, using proper body mechanics and energy saving techniques to better perform mobility tasks.

Occupational Therapy promotes independence with daily living activities like bathing, eating, dressing, self-care and home management tasks. Participants gain strength and coordination while learning to conserve their energy.

The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.

I came out of Indianapolis hospital with a spinal/neck injury. After 2 + months in hospital I rehabed elseware in indy for 2 weeks. Thats all Insurance allowed. I had to go to Trail point Village until my ramp was built which was going to be about 2 months. After day 1 I knew I was in trouble. The bed was not proper for my back and it caused the incisions in 2 places to open up. I tried tell them that only certain bed would work and my prior rehab told you my needs. They said these are only beds we have. They offered no womb care and back was leaking all over the sheets. Day after day I saw and seen what was happening to me was happening to others in there circumstances. When you speak up you are crucified by staff. My spinal injury caused my bowels to be uncontrollable. I realized I was doing everything myself because they would not come to help in any way. I took it for about 3 weeks. I then called and got temporary ramp built, bed and wheelchair myself thru Alicks delivered to my home. That was alot money I am still paying on credit card to this day. Trail Point was no help on getting me the items ordered either. I was determined to get heck out. I will say one thing positive about this place. It sparked a fire in me to say I'm doing everything here on my own, then maybe I can make it on my own at home. I have sworned to my aunt, I will never go back into that situation. They humiliate, degrade and talk about you/others in hallways and laugh. I will take my life before I go back to that again. Those other patients in there are stuck and can not complain for fear. I complained and saw what happens when complained. I hope this post helps this place to get right. I hope this post tells others to be aware of where you place people. I was there July of 2024 to August 2024. Sorry for the late review. I've been battling spinal and womb infection since. I now have it to spinal/bone infection. I want to voice for those who can not speek for themselves...Thank you for your time.

Our almost 35 year old daughter just spent 30 days at Trail Point so that other Mom and I could have a much needed respite. Our daughter is not your average 35 year old, she has Spina Bifida, paralyzed, has a colostomy and urostomy…all in all a fairly complicated patient. The staff was AMAZING and this is coming from a Momma Bear retired Respiratory Therapist. Every member of the staff was incredible and interacted with the residents…housekeeping, dietary, CNA’s, nurses and admin. We will be doing all of our respites here for sure ❤️ This is our daughter at the Halloween party dressed as Melissa Etheridge 🥰

My brother was sent to Trailpoint Village after a stay in the hospital for rehab. The staff was really super caring for my brother. They kept me in form of all his problems that he was having. And really was helpful whenever we asked for anything when we were there for a visit. I know if we ever need rehab again, Trailpoints would be our choice.

SOME of the nurses are awesome some shouldn't have a license of any kind. Administrative staff is less than competent. They told memorial hospital before they accepted my transfer that they had all the supplies and equipment that I need to live. Gtube extensions, food supplements, meds , etc etc. I got here and they had nothing. I had to get all my food supplements and tubes from my house. It took them 6 days to get any of those things. So if not for bringing my own food and supplies I would have not eaten anything for 5-6 days. And as for my meds, they were confused about that also. It's not approved yet but soon. I heard that for a day or two. So if you're coming here or sending a loved one. Good luck. It's very hit and miss with the nurses. Here's an example of the way things are here. Posted as a non smoking facility. ONLY FOR THE RESIDENTS Administration and all others are allowed to smoke. Outside only of course but none the less. That's how the administration runs this FINE business.
