My 97 year old dad was there 8+ weeks for physical and occupational rehabilitation following a bout with COVID 19 and 2 week hospitalization. While the CNAs and nurses were generally pleasant and professional, staff turnover was extremely high, staff morale was horrible and it appeared to be under staffed. I was there with my dad every day spending hours and only saw his physician once. The shower was never cleaned and my dad was never showered the entire time he was there. He got C-Diff while there and was discharged to come home by the facility’s physician even though they (RN and doctor) knew he was very sick. Within hours of being home he was transported by ambulance to the ER, was admitted and died 10 hours later. He was a vibrant, mentally sharp person despite his age even after recovering from COVID. I do not recommend sending your loved ones here.

I give this place five stars of 10 out of 10 the place was very clean. It smelled nice. The staff was very respectful. The food was great. I really enjoyed my roommate that I shared a room with my experience was great. I really don’t have anything bad to say about the place. I recommend this for anyone that needs to be in a rehab center or needs to get help in the future, but my best overall experience was the intake manager. Brittany runs the first day. I met her when she did my intake. She was so nice respectful understanding there’s nothing that I needed that she did not help me with her smile brighten my day she come and checked on me every day to see if I needed anything to help me heal and recover, as well as on my journey to go back home. She was the best part of me being there thank you Brittany for everything that you did and for making me feel better and like I was at home.

My great aunt has a private room, which has been great for her comfort and privacy. I’ve met some really awesome people during our visits. We did have a small issue, but it was taken care of quickly, which I really appreciated. A special shoutout to Nurse Beck, who was very helpful and kind. The woman in admissions was also attentive, welcoming, and made the process much easier for our family. The food could be a little better, but overall it’s a nice facility with friendly people who genuinely seem to care. I’m very satisfied with our experience and would recommend it to others. 😊

I recently visited this nursing home both as a family member inquiring about care options and as a delivery driver, and I was genuinely impressed with the experience. The facility is exceptionally clean, well-maintained, and has a warm, welcoming atmosphere. During my visit, Britney was incredibly kind, professional, and helpful. She took the time to answer my questions, provide information, and made me feel comfortable throughout the process. Her compassion and dedication truly stood out. What I appreciated most was how personal and attentive the staff are with residents and visitors alike. It’s clear that they take pride in creating a caring environment where people feel valued and respected. Based on my experiences, I would highly recommend this facility to anyone seeking quality care for a loved one.

Not the best at communicating- A handful of nurses are fabulous, but most lack care and attention. They also gave my 82 year old father COVID. My father left the hospital with an abdominal blockage. My father was tested numerous times for covid and he was negative upon his delivery to Optalis. When I drove him from the hospital to Optalis- Nobody was waiting for us. They basically opened a room and shoved HIM IN no paperwork. No transfer of his prescriptions which took days to get. That part was terrifying. After my father was there for 5 days, they brought a man in with a hip replacement surgery as his roommate who brought covid to the facility. He gave it my Dad. WHICH ONLY MADE MANNERS EXCEPTIONALLY WORSE! WHY WOULD YOU NOT TEST FOR THAT BEFORE BRINGING SOMEONE INTO A PLACE LIKE THIS? Overall not a structured place at all. Nobody knew what was going on day to day. The place was clean. The food was less than what you'd expect in a prison . The service terrible. I was lucky to get a person answering to the main phone number. I will say There are a few nurses that were exceptional and went above and beyond! But most EVERYONE acted like they could not be bothered. Nobody really had a sense of care or genuine attitude whatsoever. I would definitely not recommend this place!
