My family has had an extremely disappointing experience with Optalis Health and Rehabilitation Center during one of the hardest times of our lives.

My grandfather passed away at this facility, and it has now been **weeks** since his passing. Our family is still unable to have him cremated because the physician has not signed his death certificate. This delay has prolonged our grieving process and caused unnecessary emotional distress.

While trying to get answers, I called the facility hoping someone could either help or connect me with the appropriate person. I explained our situation to the receptionist, Theresa, including the fact that it had been weeks since my grandfather passed away and that we were still waiting on the death certificate to be signed. Rather than being met with compassion or professionalism, I was treated very rudely.

I understand that the receptionist is not responsible for signing death certificates. However, when I asked to speak with a manager or someone else who could assist me, I was transferred to a voicemail, and my call ended without anyone addressing my concerns. At no point did I feel that anyone made an effort to help or even acknowledge how devastating this situation has been for my family.

No grieving family should have to spend weeks waiting for paperwork that prevents them from laying their loved one to rest while also feeling dismissed when they reach out for help. Compassion, communication, and professionalism should be standard—especially after a resident passes away in your care.

I hope Optalis investigates both the prolonged delay surrounding my grandfather's death certificate and the way my family was treated when we reached out for assistance. Families deserve timely communication, accountability, and empathy during one of the most difficult moments of their lives.

I’m staying here not because I need any kind of physical therapy but because when I was discharged from Fairfield Medical Center I had no place to go.
The staff has been very supportive to me.

I have a lot of good things to say about Optalis Canal Winchester. The facility is clean and smells good. The staff are really friendly and truly do care about the people they are serving. I am on my second visit because I broke my ankle and need rehab. So nice to remember so many faces. It makes it easier when going to a nursing home. So glad I came back to the same place. Everyone asks do you remember me and of course I do. Especially my favorite aide, Andre, he has so much compassion and empathy he treats me with respect and kindness. He always takes good care of me. The only complaints I have are when you first check in let’s say Thursday early around noon , they were sent all your meds that morning but they don’t have your meds till around 3:30 am or 4:30 am the next day. There is an emergency drawer the nurse has to get permission to open and get an emergency pill for you till your medication comes in the middle of the night. They only will give you 2 pills in around a 16 hr period. I should have had 4 pills in a 16 hour period. So it’s difficult and uncomfortable with a lot of pain. I can see waiting 4 hours for prescription to come in but waiting 16 hours, just because the headquarters in another state where the company pharmacy is located as well decide all locations would use the company pharmacy whether it’s in state or not. So we are punished in Canal Winchester Optalis location because pharmacy is in another state. Just seems wrong to me, they should at least supply enough medication in emergency drawer. Also nurses came across as scared to call pharmacy to get emergency medicine. This is my second time at this and leaves a bad experience when you first come. I can’t think of any more negatives and it’s mostly positives.
