Eastland Healthcare and Rehabilitation Center has been serving the Columbus community for over 25 years by providing loving care to our area residents. Our staff makes it their mission to reach out and care for each individual by providing 24-hour skilled nursing services, a full complement of rehabilitation services, and loving attention to the needs of our community's senior population. Whether you need a short therapeutic stay after a hospital visit or long-term care, Eastland Healthcare and Rehabilitation Center is a community of compassion and activity. We are proud to offer specialized care and programs for our residents with dementia. We would love to spend time with you! 

Our Story

Who We Are

Guardian Elder Care is both our name and our philosophy. We strive to make Service and People the pillars that set us apart in the delivery of skilled nursing and rehabilitation services in our communities.

Guardian Elder Care is a privately owned Healthcare Organization providing services to communities in Pennsylvania, Ohio, and West Virginia. Today, Guardian has over 4,500 skilled nursing, personal care, and independent living units.

Since we began in 1995, Guardian has grown to 58 Healthcare Communities, with 51 located in Pennsylvania, 5 in Ohio, and 2 in West Virginia. Guardian also operates Ancillary Divisions to provide Pharmacy, Rehabilitation, Advanced Practitioner Services, Home Health, and Home Care Services. These divisions were formed to provide excellent support to our patients' communities and neighbors in the surrounding areas.

As Pennsylvania's largest independent provider of skilled nursing services, we are proud to be a recognized leader in offering healthcare services to today's seniors.

Our Philosophy

Guardian Elder Care's Core Philosophy is to operate facilities in rural communities that can be supported geographically by its Leadership and Ancillary Teams. We recognize that efficient facilities need a certain level of organizational support to capitalize on the economy of scale. That support is driven by Guardian's Senior Leadership team which has over 175 years of cumulative experience in the Long Term Care Industry.
