Rehabilitation Care

Physical, Occupational and Speech Therapy Services

Our primary goal is to get our senior rehabilitation patients back home as quickly and safely as possible. Saber's skilled team of therapists is unsurpassed. Whether you are improving general mobility with physical therapy or rehabilitating from an injury, stroke or surgery, our professional staff will facilitate the best treatment plan to achieve your highest level of wellness.

How do we do that With expertise and a smile. At Saber, we are not only rehabilitation experts but cheerleaders, too.

Physical Therapy

Our physical therapists (PTs) assist patients to regain strength and balance, prevent falls, increase range of motion, decrease pain and regain functional mobility. This all contributes to rebuilding their confidence and regaining their independence.

Physical therapy also helps patients increase their strength, muscle function, coordination, endurance and mobility. All of these are designed to help patients win back their independence, health and safety.

To get patients home as quickly and successfully as possible, Saber physical therapists utilize our exclusive Progressive Approach to Home (PATH) program. PATH includes customized treatment plans for each patient, and, as part of your discharge process, a member of Saber’s physical therapy team will conduct a home evaluation to further ensure a comfortable, safe and successful return home.

Occupational Therapy

The activities of daily living are at the heart of occupational therapy: bathing, dressing, cooking, using the restroom and driving.

Saber’s occupational therapists (OTs) help patients learn how to safely and independently perform these daily tasks, providing peace of mind for both patients and their families.

Saber’s occupational therapists help patients return home quickly and safely by utilizing our exclusive Progressive Approach to Home (PATH) Program. Saber OTs provide customized treatment plans and exercises to improve a patient’s strength, endurance, range of motion, coordination and balance, as well as therapeutic activities for memory and cognitive skills.

Our team also helps patients learn how to adapt their daily activities and modify their environments so that they can function better at home. Our PATH program includes a home visit by a member of our occupational therapy department and recommendations for home modifications to facilitate a safe and successful transition home.

Your safety is our mission.

Speech Therapy

Speech therapy goes beyond the patient’s ability to speak clearly. Saber’s speech pathologists (STs) assist patients whose conditions have led to communication problems, memory loss and swallowing difficulty. Speech therapy can help patients better comprehend written and verbal communication and improve their ability to reason and orient themselves. For patients with swallowing disorders, speech therapy also addresses oral muscle strength and function for safe eating and drinking.

Using our Progressive Approach to Home (PATH) program, speech therapists work with patients to find effective ways to improve cognitive function or compensate for barriers. A member of the speech therapy department can be present during your on-site home evaluation, so your transition home can go smoothly, efficiently and safely.

Skilled Nursing

Our clinical success includes getting rehab patients home quickly and safely through our Progressive Approach to Home or PATH program. In pre-discharge PATH meetings, both patients and families can expect to be involved in transitional care and discharge planning. Additionally, we partner with the Patient Education Institute to use its X-Plain system, a multi-media library covering thousands of health topics. This provides excellent resources for ongoing patient and family support and education, so patients can remain healthy at home.

Updated review: 7/1/26<br><br>My 26 year old quad son stays here. I’m here everyday- he stays on the back of the GEM unit and his dayshift nurse Jasmine is amazing and an asset and irreplaceable part of the facility and well as his night shift nurses Laura and Taonna,Pearl and Brynne.<br>Sharita and Renada are also nurses that take amazing care of my son. They unfortunately use not so good nurses on the days that Jasmine isn’t here.<br>As far as the CNAs go- dayshift Marinah,Marquetta, Lori do an incredible job and always check in on him. Nightshift CNAs Steve, Kam, Tammy, T and Tiny are absolutely wonderful and reliable!<br>The RTs make sure my son can breathe! So huge shout out to Cindy, Brigid, Jay, Todd, Joe and occasionally Jasmine and Gabe!<br>Thank you to the DON and the ADON for trying to put the standards back in the care being given here! The administrator is very lucky to have a great team behind him to help guide him with all that goes with running a SNF!<br>Thank you to folks in the kitchen and in Laundry and Housekeeping and this place couldn’t run without the help of the maintenance team! And thanks Zach for always trying to make sure things that are needed are ordered and stocked.<br>Only downfall is I don’t think they have 24 maintenance- bc the nightshift CNA Steve usually has to fix things in the middle of the night.

Right at the moment, I’m giving them a one until I see them improved and how they take care of my brother. They call and leave a message with you and they never are there to pick up the recall.<br>They say he’s falling out of bed and I wanna know what they’re doing to keep him from falling out of bed.<br>And they’re not a very hygiene place because hes got a trac tube and they don’t keep it very clean and he ends up back in the hospital with pneumonia.<br>This has happened twice already. And I’m looking around to see who I can call professionally to complain about this.
