We had to visit someone after 8:00 PM who had just been transferred to the facility earlier in the day. We need to bring fresh clothes (underclothes) since the patient had been in the hospital for over a week. The evening staff were super nice!

I was extremely disappointed when I arrived at Autumn Lake Healthcare at Patuxent River to visit my sister. She had just been discharged from Johns Hopkins after being in a medical coma for three weeks and recovering from several newly diagnosed medical conditions. As a result of her illness, she experienced memory loss, mobility limitations, and other ongoing medical issues. This entire situation has been a nightmare for our family. After being hospitalized at Johns Hopkins for approximately a month and a half, our family selected Autumn Lake Healthcare at Patuxent River for rehabilitation. Unfortunately, our experience has been deeply concerning. I believe that on the night of her arrival, my sister expressed that she wanted to return to the hospital. The facility appeared unclean, and there was a noticeable odor in the hallways upon entry. It was a constant struggle while our sister was admitted there. We had to call multiple times to address concerns about her care. Staff frequently did not answer call bells when patients needed assistance. When the family raised concerns, staff appeared dismissive and unresponsive when family members were not present. On one Sunday visit, we requested supplies to assist in caring for our sister and were informed by a nurse that they were out of supplies and might receive more the next day. However, my sister and I walked through multiple wings of the facility and were unable to find any clean towels or wipes in the supply trays located in the halls. The overall condition of the unit appeared disorganized and poorly maintained. The following Monday, I contacted the manager to report these concerns. She stated that staff may not have provided accurate information. I explained that we had personally checked other wings and confirmed that supplies were unavailable. I also informed her that three individuals visiting that Sunday were mandated reporters and that a formal report could be submitted due to the observed conditions. During another visit, I witnessed a patient walking out of her room with feces running down her legs and into the hallway as she walked toward the laundry area. Staff members were standing at the nurses’ station and did not intervene or assist the patient. On several occasions, I observed nonchalant behavior from staff when patients requested assistance. Additionally, there were serious concerns regarding organization and quality of care. A lab order I personally provided to the nurse for my sister was misplaced, and her medication refill was overlooked. These repeated issues demonstrate a lack of attentiveness, lack of empathy, poor organization, and inadequate leadership within the facility. Based on my experience, I would not recommend this facility to anyone seeking care for their loved one.

This is the first day at the facility for my family member whom was placed here for rehab after a hip replacement. It is very clean, staff seem to be friendly. My only complaint is the fact that the staff takes so long to respond to the patients. My family member whom is of sound mine had a BM and and rang the bell at 4:15. It took someone 15 minutes to respond for us to be told that someone would be in to take care of him. I waited 15 more minutes b4 I had to leave. I had a 25 minute ride home and he still had not been cleaned at 5:15. He rang the bell again and still no one came. At 6:20 he was told that they could not clean him up during dinner time. At about 6:45 the doctor came in and requested that his bandage on the buttocks be change and told them that he had to cleaned up. An aid came in and realized that he needed to be cleaned up and mumbled that she wasn't going to clean him and told him that she had to get the charge nurse to come and change his bandage and clean him up. The nurse finally came by 6:55 to take care of his needs. It had taken almost 3 hours for his needs to be taken care of. This is unsatisfactory and unprofessional. They are not taking in consideration of the patients dignity. I hope that this gets better. Because, I do not want my family member or anyone's else's family member to have to wait that long to be tended to. That is the reason the patients ending up with infections and bed sores. If they do not want to do their job to take care of these patients/clients, they need to find a new line of work.

Update : Given our bad experience the center was quick to satisfactorily respond. No place is perfect but it’s important how they act on complaints and so far I’m very pleased. Changing the rating from one to four based on their response. My mother was admitted yesterday evening for short term rehab and it was a horrible experience. When she arrived the room she was assigned was dirty, the bed didn’t work, and had no sheets. When they finally got it to work the staff acted as though they were being inconvenienced. We left her there with a very uncomfortable feeling and unless they can somehow redeem themselves, we’ll be looking for a new facility. Also the hallways were a mess and smelled like urine and the nurse on duty was absolutely NO HELP. I could go on and on but I do not recommend this place based on this experience.

I'm a food delivery driver who came to the facility to drop off food to a patient. There's no sign up that says "When you push the button, wait for somebody to buzz you in". It just says to push the button after hours. I push the button a few times then check the doors and they were still locked, so I pushed again and a lady screamed "OPEN THE DOOR". The yelling was crazy. Then I go and deliver the food to the patient and try to get out and I'm locked in. It says to push a button after hours to be released. I push the button for well over 25 minutes and absolutely nobody let me out. I kept checking the doors and I'm locked in. The staff completely ignored me. I was one step from calling 911. I'm a civilian and it's not a joke to entrap somebody in a facility. I pressed that button for at least 15 minutes. It also smelled awful in there so I had to breath through my mouth. Finally a lady comes to the front and instead of saying sorry she says oh you're locked in after a certain time. I told her I've been pushing the button for 15 minutes and there's a little camera so I know they saw me. I told her I was about to call 911 and She starts to yell at me. I put my hand up and walk away to my car as she continued to yell. That was a nightmare. I would never feel safe leaving anybody at this facility. They need to be shutdown. My clothes still have the smell in them, and I can't imagine as a patient trying to get help. That's terrifying. I'm going to pray for those patients. 🙏❤️
