My father was admitted to Bradford Oaks for rehabilitation, and our family’s experience was deeply disappointing. The facility’s cleanliness was far below what we expected from a nursing and rehabilitation center. There were ongoing concerns about sanitation and the overall condition of the building. Maintenance issues appeared to go unresolved, including problems with toilets and basic facility upkeep. What concerned us most was the apparent lack of urgency in addressing resident needs. Families trust facilities like this to provide safe, dignified care for their loved ones, and we often felt that standard expectations for care, communication, and resident comfort were not being met. Rehabilitation facilities serve vulnerable people who depend on staff and management to maintain a clean, functional, and caring environment. Based on our experience, Bradford Oaks fell short of those expectations. I encourage families considering this facility to visit in person, ask difficult questions, and carefully evaluate conditions before making a placement decision.

It was a pleasure to visit. Bradford Oaks is clean, didn’t have a bad odor. Residents seemed to be happy and in good spirits. There was a Hawaiian luau going on in the dining area, with a host from Hawaii and the women had a lei & a grass skirt on, the men had a lei and hat on from Hawaii! It was festive and adorable. My husband and I only heard 1 complaint, the food needs seasoning, otherwise everything else is delightful there!

I am overall satisfied with the care my mother is given. The staff is all very caring and tend to her needs very well. However there is a couple things I would change. 1. Her room is to hot most of the time I am in there. That can cause dehydration in the patients there. If a patient is bed ridden, unable to walk unless staff comes in to adjust it or someone says something. It stays the same. 2. Dietary, I know everyone has dietary needs. The variety of foods need to expand. Please think outside the bun. Perhaps do a cheese pizza day or for breakfast add some fresh fruit something other than eggs. 3. When family comes to see a patient over an hour away it would be nice to accommodate that family by allowing them to sit in during a 3 hour dyalsis session while wearing a mask. It is very inconvenient to have to drive home an hour an 20 mins after just driving that long to get there. Policy changes should be considered in that area. Otherwise I appreciate that Autumn lake seems to be well kept, with great medical staff. They all seem very nice. They deserve 6 stars. Thank you for taking great care of my mom.

My brother’s in Discipleship 2027 class and myself we had a very lovely warming, and God filled time today at autumn Lakes. It was a pleasure to come and to see the faces of the residence there and to see their enthusiasm and to feel their heart it was truly a blessing.

Good Evening, there’s this one CNA on the north side unit. Her name is Mia and she’s the best worker there in my opinion. She has a genuine soul and a big heart ❤️! She needs a raise!! Anytime I come in and I see that little girl I know my brother is in good hands. God bless her 🙏🏼✨🕊️
