This facility deserves every one star review it receives. It's genuinely shocking the limits of incompetence, accountability, and subpar care this place and its staff can provide. When my father was being discharged we were told that this was the only option for skilled nursing we could send him to, but the low ratings left me concerned. Within the first day of his arrival we knew allowing him to be transferred there was a mistake. Every single staff member would respond with "that's not my job", "I'm not familiar with his chart", or "it's my first day here" whenever asking for the most basic of medical updates. In fact - we did not receive a true medical update for an entire week despite our loved one being given a severe prognosis prior to his arrival. The only communication we received was from case management who was prematurely ready to discharge him. We had a family meeting where we were told it was just a conversation about making a plan for next steps towards discharge, despite our objections for the conversation given that we hadn't received any medical updates, so how could we possibly consider a discharge? No exaggeration - not a single member from the nursing staff came to the meeting to give us the update we were begging for. Instead, case management actually said "we aren't talking about a discharge today, tomorrow, or even next week!" and was pulled from the room by a director, then returned to say that actually his insurance was denied and my dad needed to be discharged in the morning. This was a result of the nursing staff not performing a single test the entire first week he was there and therefore reported he was "a healthy 64-year-old-male" to the insurance, who then yanked the approval. We then were panicked and pressured to paying over $6k upfront for more time because my father was still not physically ready to go home. The discharge went equally as terrible, none of the equipment that was promised was prepared, none of his prescriptions were faxed to his requested pharmacy and then required a prior-auth that led to a lapse in his treatments. The icing on the cake was that the home health company we were told approved his care actually was denied, so he was discharged home with only a few days of medications and no scheduled medical care going forward. My family's experience at CareOne was a cautionary tale of what's wrong with insurance and healthcare in this country. Even if you feel like you have no other options - DO NOT send a loved one here.

My dad was admitted here from the hospital. He has dementia and other issues. We’ve been to other rehabs with him, and CareOne is like a 5-star resort in comparison. The staff is amazing - attentive, detailed, caring, communicative and consistent. The room is clean, spacious and bright. It is not depressing here.

My 92-year-old mother was admitted to rehab after a brief hospitalization at SSH for a left-sided stroke. I live in New Jersey, but I'm closest to her and have been involved in her care. My only contact with the staff has been by phone, and every single one of my interactions was unpleasant. The very first call I got was from the discharge planner, who was incredibly abrupt. I tried to talk to the charge nurse for the unit, but I left three messages and didn't receive a return call. After my mother started getting bills at home (which she shouldn't have received), I left 2 messages for the administrator and 2 messages for billing, and didn't receive a call back. The only calls I did receive were from a nurse who wanted to put my mother on opiates for pain, without optimizing her non-opiate pain meds. I requested an update in 2 days, but of course nobody called me back. Since my mother is only partially verbal, she can't complain about the lack of attention. I can. I'm a physician, and I can tell from 300 miles away that CareOne Weymouth is suboptimal. I'm only glad that I got her transferred to another facility.

I truly wish I could give 0 stars. Please please please do not bring your loved ones here you’re better off hiring a strange homeless man to come in and take care of your loved ones in their own home they would get more care that way. My grandmother is 93 years old and I say this in the most non-biases way is the sweetest, funniest, loving woman she was a director of nursing in nursing home facilities her whole life so she knows how they should be run. She came in for short term rehab for a shoulder injury and was treated with nothing but disrespect, elder abuse, and down right awful behavior. They staff here has forced her to use a bedpan because “I’m too busy to keep taking you to the bathroom you take to long to walk in there” -CNA as it’s a few feet from her bed… she has been refused her meds… they forced her to choke on her food by laying her bed flat down while she was chewing… the nurse laughed in her face and told her the issue was her own fault when she asked for her blood pressure and anxiety meds that were way over due I’m talking 24 hours over due when her blood pressure was 199/79 with a HR of 133 and when my spouse confronted the nurse in a kind manor he laughed at my spouse and the director of nursing then said my spouse was “intimidating staff” and he was asked to leave and the nurse was not reprimanded they did not even take the time to question the people who witnessed the disagreement between staff and my spouse they asked us to leave to silence the elder abuse… when I said I wanted to file a complaint the man came to take my complaint and the director of nursing rushed him out so I couldn’t file… my grandmothers Roomate was so concerned in this place she looked at me and said “be careful what you say to these nurses if they don’t like your behavior I wouldn’t put it past them to over administer drugs to end your grandmothers life I’ve seen it happen before in places nicer than this one” no patient should have to be in fear for their lives to speak up for themselves…
