Very clean and modern-looking facility. Compared to other nursing homes I’ve seen, this one feels especially welcoming and comfortable. The hallways are fresh with no odors, which really stands out. The staff are polite, friendly, and make you feel at ease right away. The food is fantastic—definitely better than expected. Overall, a warm and well-maintained place

My brother was recently discharged after a two week rehab stay for a knee replacement. The rehab team was extremely knowledgeable and attentive to his needs. The food was great and the nursing staff was so kind and helpful! The whole process the admission to discharge was seamless. I highly recommend this facility! Thanks to all the staff, we appreciate you!

My mother stayed here for approximately 3–4 weeks, and the experience was extremely disappointing and concerning. Basic patient care and communication were consistently lacking. The food was poor, and repeated requests for alternative options were ignored, especially during the days leading up to discharge. During the final days of her stay, she did not receive therapies as expected, and there was a noticeable lack of empathy from staff. The discharge process was completely disorganized. The family was not informed of the plan, transportation details were unclear, and there was no coordination with us. I requested updates multiple times with little to no response. I did not receive discharge paperwork, until 5pm. There was no clear medication plan for when she arrived home, and even her own insurance company contacted me because they were not notified of her discharge until hours later. Communication with leadership was also problematic. The head nurse on Cardinal Unit 2 was rude during phone calls and unprofessional in her tone. Overall, concerns were dismissed rather than addressed. In addition, I let her know I could tell when she was muted me because I could tell when the phone went silent. When I stated that the phone stopped with silence breaks. I had to request her meds to be called in at 5:30pm of discharge after she was home 5 hours prior. The nurse stated to me that she is past her shift time to deal with this and she said "you don't trust me" to call this in. I did relate this could have been done earlier in the day. Most alarming were lapses in basic care. On several occasions, my mother sat for up to three hours without being changed, remaining in urine and feces. Some staff members were unpleasant and at times yelled at her without cause. Access to the patient portal also took two weeks despite repeated requests. Also, there wasn't much practice for my mom to use the bathroom on her own. Or to transfer herself to and from the bed to ensure that she was ready to go home. I work in healthcare and was able to navigate the system, but I am deeply concerned for families who are not familiar with it. There was little support, poor coordination, and a lack of accountability. I will also be reporting this experience to the local Ombudsman. I also would not put any stars on this rating. But I had to be able to post.

This was my first experience at v a rehab facility. The experience was something I wouldn't want to go through again. The 11pm to 7am shift was a nightmare literally Iwas a two assist non weight bearing due to gout. I was there because of a thyroid problem I used call light to get assistance to rest room no one answered light for at least 45 minutes to an hour aide finally came in I said I needed assistance to restroom the girl said we'll go ahead do it I said I needed help she walked away without saying a word had no badge did not know her name came back quite awhile later put me in restroom didn't help transfer to toilet. I pulled call light when done no one came took me 45 minutes to get myself to sink to wash hands and back to bed . At 6:15 am the 11-7 shift was running around changing people, cleaning. They came in at that time and took care of my roommate by changing her diaper was so heavy when hit floor was awful. They also left her in feces for over two hours in room where they watch TV. Also was forcing her to take meds when she refused. They brought me wrong meds and could not tell me what meds they were. When I tried to say something to 7am -3pm shift they just covered for 11-7shift. Roommate was taking out by daughter because of neglect. My daughter ended up taking me out for neglect. I ended back in hospital 2 days later and went yo another rehab which so far is much better than the one care concord. I was also in medical field for about 30 years... Good luck if you go here and please watch your family member very closely.. thank you for listening God Bless. PS. Don't even want to give one star. So the facility asked me to call them to discuss problem which I did Carol the administrator answered I told her that the facility asked me to call to discuss the issues and she said to give her a minute I said okay all of a sudden the phone is ringing and it went to a voice mail with a man saying carol the administrator to leave a message. I called back and it didn't even go to the administrator it supposedly went to a floor and the girl said I have to transfer you downstairs and it. just kept ringing. Very unprofessional. So I spoke to the administrator Carol she was very nice and understanding she stated there was stuff they had to work on with staff. She was unaware of all the issues on night shift she assured me she would look into the situation let's pray she does... Really look into where your sending your family member they should be cared for the best way possible for what ever there going through. God Bless
