Had family here at this facility. The respiratory team was fantastic with them. You can tell that team absolutely cares about theirs patients and is proactive about their care and needs.

My grandpa came here with me, and it ended up being a really meaningful visit. He served during Nam, and places like this always make him stop and reflect on the people, memories, and sacrifices connected to that time in his life. Walking around Monument Health Canyon Rim gave us a quiet moment to talk, remember, and appreciate how important health, service, and community really are. The staff and atmosphere were kind, respectful, and welcoming. It meant a lot to be here with him, and I’m grateful for the experience.

⭐⭐⭐⭐⭐ Monument at Canyon Rim has an outstanding team. Mike and Dawn are wonderful to work with, and their collaborative approach makes coordinating care seamless. Their care team is compassionate, professional, and truly dedicated to supporting residents and their families. I recently worked with them to help a family navigate their loved one’s care, and the communication and support they provided were exceptional. Highly recommend Monument at Canyon Rim for their commitment to quality care and teamwork.

If i could give negative 100 stars, i would! I was a patient here and have the same feedback and the most recent post. This place should be renamed Monument Hell. I’m at a loss for words about the state of the place. It’s a health hazard. For a facility meant to be a place of healing, it’s incredibly unsanitary. The floors, walls, toilets, sinks, and overall space are disgustingly dirty. Now, let’s talk about the staff. It took 27 minutes for the nurses to come to my husbands room. The staff treated patients terribly. Some of them sat around talking and laughing while a patient, in no underwear, begging for help to open a brief bag. The poor patient had an odor of feces and clearly had no support cleaning himself. They left my husband in bed for days, neglecting bathing needs. They did not brush his teeth and even forgot to bring him meals. My husband could not move around independently and when he asked for help, the staff told him to stop requesting their support. Most of the staff were disrespectful and could care less about their tasks. There was not physical therapy or occupational plans and no routine for patient care. I don’t know who checks off supports provided to patients, but when I asked to see a checklist or a plan for support, they did not share any paperwork. I’d love to see what they share with state and federal governments for documentation. I’m not sure where they find these people to support this field of work, but many of them lack the knowledge, kindness, or skills to be in this profession. Please don’t send your loved ones here. You will 100% regret it!
