My Mom was here 12 years ago. We were on watch 24/7. I walked in one day and she was naked from the waist down and I asked for clothes to be put on her and the aide said it was easier clean her

Please do your homework before placing anyone here. Do a Google search on this place and find public records! It will SHOCK you to your core !! My loved one was here at the pavilion about 2 weeks and was sent to the hospital from a fall . My loved one recieved 4 stitches above her eye and a fracture to the arm. The nurse told me my loved one fell out of bed and sustained these injuries at 89 yrs old !! These injuries could have been prevented if they would of taking the time to read her chart. Everytime I go to visit my loved one this place still hasn't put any a mat on the floor by the bed, lowered the bed, or put a bracelet on my loved ones wrist or outside the door alerting workers about being a high fall risk. I've found my loved one another facility to move to but the Administrator and Social Worker at this one keeps hindering this. But my loved one WILL be moved !! Please read up on this place YOU WILL BE SHOCKED this is no run of the mill place !

A Wonderful Experience from Start to Finish My family and I had an amazing experience with Grande Oaks Rehab Center. From the moment we arrived, everyone was kind, welcoming, and truly cared about our needs. The staff went out of their way to make sure we felt comfortable and supported every step of the way. The nurses, therapists, and caregivers treated us like family, and the management team was always professional, attentive, and ready to help. The facility was clean, peaceful, and full of people who clearly love what they do. We’re so grateful for the excellent care and compassion we received. Grande Oaks Rehab Center is truly a special place, and we would recommend it to anyone looking for a team that treats you with kindness and respect.

I came to visit a relative at Grande oaks facility. Upon entering the strong smell of urine and feces in the air was overwhelming. While there, I've talked to other residents who escorted me to their rooms to show me the disarray of clothes, all over the floor and broken furniture, dressers, and drawers, piled in corners of the residential rooms. One resident showed me they didn't have running water from the fauces in their room. After trying to speak to the staff which were very rude, and condescending. I decided I had to leave, I couldn't take any more of the stench of this facility any longer! Frankly spanking, I've been in dog kennelscleaner and more presentable than this place!

Beware beware this place is the absolute worst facility in Ohio complete and utter negligence. They left my dad choking in his room and the lights off. I showed up, walked by the nurses station. They stopped me and said that he aspirated during lunch didn’t call a doctor. I walk in the room. He was half dead I yelled for them to call 911 they did not. They proceeded to call a physician team that took 30 minutes to arrive while my dad pulse oxygen was at 68% it was so low he was actively receiving brain damage because they didn’t want a make report if they called a real ambulance. I didn’t know they didn’t call 911 I was bagging my father to try to get him oxygen. Cannot believe this facility is still open! Most of these reviews are fake so don’t let the 3.5 stars deceive you there is a separate review system that the nursing facilities use internally and they are about a two star. Please read this and I hope it saves one of your family members lives. I would post pictures but it was very graphic walking in to him suffering and gasping for air
