[If anyone happens to read this, do yourself and your loved one(s) a favor by never bringing them here. Neglecting a new admit.. I can’t imagine what those that are and have been here go through.]

Y’all really don’t even deserve a singular star. My loved one wasn’t even at this place for 24 hours before he had to be taken to the ER and admitted all because the people on morning shift dismissed his calls for help. He tearfully informed us that he was trying to get help because he felt like something was wrong, and no one came to his aid until it was to the point that he needed to be transferred to the ER. He could have passed away. He won’t be back here ever again. Thank God he’s ACTUALLY getting help from a hospital because this place clearly didn’t care enough.

If I could give this facility below 1 star I definitely would. My father was discharged from a hospital to this facility for rehab after almost a 1 month stay. Within 24 hours he was admitted back into the hospital. My father called out for help and rang his call bell constantly for hours. His call for help went unanswered and so did the call bells that were constantly turned off. During shift change he briefly seen the nurse. Tried to explain his distress just to be told she would be back and never returned. The next shift came in and noticed hours later that he was in respiratory distress and then he was sent back out to a hospital. If you can disregard a new admit, I assume you really don't care about the current patients. DON"T SEND YOUR DEPENDENT FAMILY HERE. That call to notify us of the ambulance ride could have a been a different horrible and traumatizing call.

Staff is shoddy and rude at best. My girlfriend of 23 years is totally blind in one eye and has only about 20% vision in the other. Needs help dressing. Staff does not or is slow to respond. Treats her as if she has no vision problems at all. One recent incident with her verbally
antagonisotic room mate caused her to leave the room for a time with staff telling her "you got what you .deserved".
To add about the so called "call button" very slow response if not ignored. Staff is indifferent (say it's not my job, say 'ok" then nothing happens. And trying to communicate with social worker nursing of staff by phone is impossible you only get voice mail which is always full these are everyday occurrences I rate this place a complete and total ZERO.

This place is horrible! Soon as you walk on the floor you smell feces. The staff is half and half. Some nasty, some nice. You call the main number it takes 2-4 calls to finally get an answer. Then whoever answers says hello instead of thank you for calling Elkins how may I help you. One person answered and said what do you want?
When you want to get inside the building the sign says call or knock but how can you get in right away if no one is at the front desk and no one answers the phone?? My dad asked a CNA to empty his pee bag and the lady said when right now? In a nasty tone. I was sitting right there to witness this. And the bag was filled to its capacity so yes right now! I could go on about this dump. I have tried to get in contact with the DON Chanyce Smalls for 2 weeks and she has not returned my calls; it took a week to even get her number because the staff didn't want me to have it. She still has yet to call me back. They gave my father the wrong medications. I could write a book on this crappy place. The rooms have shared bathrooms with the room beside it. My dad's bathroom door was open and someone from the other room went into pee, and I didn't know who it was. They left feces in the toilet. The entire place is out of date and filthy. A patient fell and it took forever for her to get help. The CNA to patient ratio is horrible. I was lied to initially when my dad first entered the facility. Then later found out the truth. DO NOT BRING YOUR LOVED ONE HERE!!!!!

This place has many residents living here. I'm sure the nursing staff do their best to care for the patients. The only thing I noticed that really needs addressing is maintenance. Such as the vents need cleaning and the walls could use some spackle and paint. A lot of the issues can be resolved with a direct approach to maintaining the place. Needs addressing
