Ebenezer Integrated Care & Rehab (EICR) is a care and rehabilitation community located at 45 W. 10th St. in Saint Paul, Minnesota. The community focuses on enhancing and accelerating recovery, managing long-term medical conditions, and helping people regain the ability to function well in day-to-day life, applying person-centered, culturally responsive approaches.
Short-term rehabilitation services include physical, occupational, and speech therapies, available five to seven days a week to support faster recovery. An interdisciplinary team of therapists, doctors, nurses, social workers, dietitians, therapeutic recreation staff, and spiritual care providers work together to promote positive outcomes and a successful return home.
For individuals with complex or long-term care needs, EICR provides daily support and ongoing medical attention. Care plans are personalized, taking into account each person's physical, psychological, nutritional, and emotional requirements to help them regain important skills and improve quality of life.
The community also includes a self-contained, controlled-access memory care unit for older adults living with dementia, offering nursing care, meals, social programs, exercise, and other services delivered by specially trained team members focused on life-enriching activities.
Located in the same building, Ebenezer DayBreak of St. Paul is an Adult Day program offering seniors daytime social activities while their caregivers take time away. DayBreak operates Monday through Friday from 8:00 a.m. to 4:00 p.m., except holidays, and seniors can attend on a daily or weekly basis.
Covered parking is available nearby in the 10th Street Ramp, with a discount voucher available at the Hub's lobby front desk. As part of a broader network, EICR benefits from cooperative care and collaboration with Fairview to support improved patient outcomes, and accepts Medicare, many private insurances, medical assistance, and private pay.
