HORRIBLE FACILITY. Management are so rude towards staff. They are disgusting they are so rude towards the staff that help run this place. Ms. Kim and Zendra have been super rude and unwilling to help and always have an attitude when asking for help. New management is needed this is ridiculous

The facility has been plagued by poor management, a lack of effective leadership, inadequate communication, and neglect of resident care due to maladministration. The in-house doctor failed to provide an individual with prescribed medication for over 72 hours without notifying administration. The facility neglected to submit requested medical documents to the insurance company, which further complicated the resident's medical care. The individual was unjustly denied access to their personal medication and requested documents at dismissal. These unethical violations not only caused a halt in the necessary medical care but also led to life-threatening medical conditions that resulted in admissions into a hospital.
11 inspection visits in the last three years, 65 citations on file. Inspections are conducted by state agencies on behalf of the U.S. Centers for Medicare & Medicaid Services (CMS).
