About Signature Healthcare at Parkwood

Signature Healthcare at Parkwood is a skilled nursing and long-term care facility in Lebanon, Indiana, with a licensed capacity of 106 residents. The community offers both rehabilitation and extended care services.

Family experiences at Parkwood vary notably. Positive reviewers consistently praise the nursing and therapy staff for genuine attentiveness and compassion, with families noting that residents regain strength, maintain engagement in activities, and feel cared for as individuals rather than simply managed. The activities program and dietary department receive specific mention for quality. However, several families reported serious concerns: unresponsive communication when incidents occurred, belongings going missing during stays, and delayed assistance with essential care needs. One family noted dietary staff failed to accommodate medical dietary restrictions, raising clinical care questions.

The facility maintains a 4.6 out of 5 Google rating across 68 reviews. While many families describe staff who "show real love, patience, and kindness," the lower-rated reviews suggest inconsistent execution in both care delivery and administrative responsiveness that warrants direct conversation with leadership before admission.

The nurturing and caring staff at Parkwood provided excellent care for my loved one. Our family is so thankful for their compassionate and attentive care throughout her stay and as she transitioned. They truly loved her like family and it showed. She was happy and well cared for until she passed. Although she couldn’t verbalize to us the quality of care she was receiving, we saw it for ourselves. Overhearing her tell a CNA that she loved her (when we stood outside the door with no one knowing we were there) and then hearing the CNA respond to her that she loved her too. She was always clean and smiling and never had a sore or wound anywhere on her. They take pride in the care they provide and it shows. They kept our family well informed with any change and updated us often. It’s a blessing for families to know that their loved one is so well cared for, when the time comes that they can no longer provide that level of care. Thank you all for all you do each and every day.

I have lived in Lebanon all my life. I have personally had friends and family stay at Parkwood for various reasons. Due to us getting older, I’ve recently had a family member and close friend stay there. One for rehab and one for recovery after a fall. I can say, Parkwood did an amazing job with my loved ones. I saw smiling facing and warm welcomes every time I came. Maybe 2 times a week. Weekend and week days. Thank you to all the staff who care and made my loved ones better. The world is a better place because of folks like them!!!

The staff are caring. Notably key people like Bianca, Lydia, Tina, Shelby etc there's a long list of people, I'm not gonna write no paragraphs like a angry ex. But the place is fine

Parkwood Healthcare is a special place with staff and resident's that feel more like family than an institutionalized facility. The care that is delivered is personal yet professional and I am so thankful to have been able to experience this wonderfully unique place.

Signature Parkwood is a 5 star facility hands down! One of the very best facilities I've had the pleasure of working with. The staff welcome everyone with open arms and truly make you feel comfortable and right at home. I would highly recommend this facility to any of my family or friends who are looking for long term care or rehab to home services. The dietary department is my absolute favorite, their manager and staff are so kind and always try their hardest to go above and beyond expectations. If you want your loved one to be well cared for in a nursing facility, be sure to send them to Parkwood Lebanon! (:
