Staff is kind to patients and friendly with visitors. The building is clean and well lit. The grounds are nicely maintained. No one wants to live in a nursing facility but this is one of the best I've ever been in.

I had a wonderful experience at Opis Riverwood in Jacksonville, FL, and I truly cannot say enough great things about the team, especially Dietary Director Lisa Albertie. Lisa leads with such compassion, professionalism, and genuine care not only for the residents but for her staff as well. She creates a warm, welcoming environment where everyone feels valued and supported. The entire dietary team reflects that same positive energy. They work together seamlessly and consistently go above and beyond to ensure residents are well cared for, both nutritionally and emotionally. You can tell that this is more than just a job for them--it's a passion. During my time there, I witnessed firsthand the kindness, patience, and dedication that goes into serving the residents each day. It truly made an impact on me and strengthened my desire to pursue a career in healthcare. I'm so grateful for the experience and would highly recommend Opis Riverwood to anyone looking for a caring facility with an incredible team.

The experience I had this evening to visit my great uncle with my father was as good as one could have under the current circumstances we all experience with family or loved ones as they age and transition from self sufficient to needing assistance as they approach the finish line . With all considered it's no easy job for staff and I genuinely can say that every employee was kind and polite with me starting from the very nice woman who guided our check- in seamlessly. Very polite and helpful. Touch screens are no small task for the generations ahead of me . Big words easily visible on a larger tablet sized check in device making typing quick and easy for those 1 finger types. She gave us the room number and clear directions to get to the rooms location. Straight there no problem . The nurse staff were all friendly and honestly the experience was very nice and we weren't disturbed at all. 10/10 regarding the staff and their interactions with us and the patients. Thanks for being the difference makers in the patients/ residents lives as they draw near to life's transition back to source. Often I don't leave reviews much less one this detailed or descriptive but I get kinda emo sometimes especially with the elderly closer to the end as life is a very precious gift that often gets overlooked by the constant struggles and obstacles to reach the goals society groomed us into seeing important. These folks needing assistance likely were no different and meeting the last stretch of life is often not met graciously by people and their families to. So to remain helpful and kind when working with very unhappy people who's freedom is being slowly transitioned can be a greater achievement than most people recognize or acknowledge making it even more difficult to retain employees. So by all means 10/10 to all the women making the best of what they have there to work with and remaining kind despite the conditions mentioned above. You women have shown what it looks like to do God's work with smiles giving Grace finest look! May God return that very same grace in your lives when reaching the inevitable stage that you're patients are in. Truly was my pleasure to meet you! Thank you, Andrew A. Milonas

My family member has lived here for many years and has mostly had great care!! There are always a few bad apples no matter where you go but I find if you communicate the problem is solved very quickly. We have had some changes for the good in caring way. Our unit manager is fully engaged and she is always available and trying her best to improve the lives and care of the patients! Our day nurse Natalie is always smiling and putting her patients first and our evening nurse Patrice runs a tight ship and again puts Her patients first. Since our new administration has come in I am very confident that my family members needs are being met.

**8.7.25** I just want to give a big shoutout to the hard work Riverwood/especially Allison, have been putting in to put our family back at ease after the experience received. One thing I can say I learned is through communication and the dedication towards love anything is achievable. Updated 8.5.25** Good morning to all. I come to Riverwood almost every Saturday to see my friend. She’s a wonderful woman …strong, resilient, peaceful, godly, and really just enjoy to be around. Just to give an update, Riverwood has called me, spoke with me, and updated me with everything in my post and on my survey reviews as I leave the facility. They have done training with their staff on due diligence on cleanliness in the rooms, turnover of the beds that are vacant, they have had a company come out and do pest control, and as for the double briefing, apparently the CNA came forward and spoke up as to what she did and took the training and information that the administrative actions did provide her with and the importance of keeping residents clean and dry with one brief accordingly. Now every time I come here, I notice a difference in the environment that she is in. She’s doing OK now. Praise Jesus! Thank you so much for your hard work!! Thank you to Jasmine and Allison most recently. I’m thankful and praising Jesus everyday for this one!!! ——————————————————— If I could put 0 stars I would but I’ll put at least one for those there who do try to do the job of the many and I mean MANY that don’t …especially the ones doing patient care like the CNAs..the backbone of the facility is completely failing!!If you have a loved one here and think they are in good hands because they are kind when you call or when you come there, you should try and pop up when they least expect it.. They cover up a lot and that’s to cover up things like double briefing or The fact that they may undermine your loved one talked down to them when you’re not around Laugh at them or ignore them say ,they do what they want When really the people that are there have diseases like dementia or Alzheimer’s and need to be communicated with compassion and heart…. if you don’t like your job You need to find somewhere else to work ——————————————————— **Update**It got worse, my loved one passed away, most likely due to their negligence . I pray God comes through. I pray for their residents that continue to be under their care. Every time I come up there….because a promise I made a promise to continue taking care of her daughter who still there …they lie,they lie …to get out of doing their jobs ..to just do their jobs, which is so simple to just take care of the people that live there that pay them to just take care of them to make sure they are clean and cared for how hard could that be? The basic needs of Maslow’s hierarchy fundamentally cannot even begin to be addressed when deception is at every corner. Have you ate? Are you clean? Are you thirsty? Are these things being done properly? Is such a simple job. They cannot do. They Cannot be trusted. I wouldn’t trust them with a 10ft pole. Here are some photos of some cockroaches of a room that they squeeze her into one night that they fabricated about a maintenance issue. All of a sudden after a lie was caught about a nurse \I recorded her doing it..the lying..and said she was a CNA because it was brought about that my friend was double briefed, and all they had to do was handle it .. she’s blind and they put her in a room that had a snack bag that was full of molded fruit and vegetables. She was blind with a snack bag if she would eat the molded food maybe it would get her sick, the bed that they gave her was full of crumbs. All of the staff was confused and didn’t know that she was moving there. It was disappointing. But forgiveness is all we can do. EAT PRAY LOVE Gena Marie
