Rakhma Grace Home provides assisted living in Minnetonka, Minnesota for individuals living with dementia, Alzheimer's disease, and other forms of memory loss. As a mission-driven nonprofit, the community serves both private-pay residents and those qualifying for Medical Assistance, GRH, Elderly Waiver, CADI, TBI, and BI waivers, accepting residents regardless of financial ability.
Founded in 1984 by Shirley Shaw, Rakhma pioneered a residential social-model of care that addresses isolation and loneliness among older adults with cognitive decline. The home-based environment provides an intimate setting with higher staff ratios designed to meet individual needs and minimize behaviors associated with dementia. Residents participate in meaningful activities like watering plants, peeling potatoes, and folding towels to increase their sense of purpose, while stable staffing and a familiar home setting help alleviate fears and restore calm.
The community's approach centers on three main pillars: promoting choice and dignity by adapting to individual needs and fostering respect and connection; advancing justice through commitment to diversity, equity, and inclusion and increasing access to services; and enriching education and outreach while valuing feedback as a growth opportunity. Care is grounded in a person-centered, holistic approach that includes residents, families, and the broader community.
Recent facility improvements have been supported through the State of Minnesota's Living Well at Home program, which expands home and community-based services for older adults. Projects have included a new elevator, exterior improvements, and program enhancements to strengthen the residential care experience.
