My Grandma was being moved to The Loft. Warren was the one who picked her up. He lead the way, then my wife, then my grandpa. Warren puleed over and stopped twice to wait for my wife & Grandpa! He then took a cart out to help my wife carry in my Grandma's personal belongings! Warren is truly amazing. I want to also give praise to Miss Paula! She was so very kind and upfront with my Mom! The Loft is where my Great Grandmother was and they was nice to when she was a resident.

I talked to the director of the store today again I also talked to him on the 19th and he has nursing on the 20th the social services on the 22nd. Larry had a bit taken into the shower and had a pretty nasty scrape skin off of his left arm because of the stroke And he had a sore on his phone told me he did have a tour her son fifth floor and the head of nursing told me that they were dating on it and they’ve been treating it for a few days. He told me if I wasn’t happy there. I could surely take my loved ones somewhere else they’re not accredited so they’re not accountable to anybody . I will be filing a formal complaint about the neglect. His call button is always on the floor. His room is dirty. I asked the lady on the floor today when cleaning comes in she said she haven’t seen him for a while. She doesn’t see him very often but she’ll try to go in and Spidey things up when she gets a chance

My sisters husband has been at this facility for years. There have been ongoing issues which have gotten progressively worse. This month all his personal items including clothes, shaver, shoes and glasses were taken. They're response was basically stuff happens! She found a large scrape on his arm and was told he also has a "moisture sore" on his behind they are treating. No idea how scrape happened. He's imoble so he didn't do it. In speaking to Greg(Director) he denies its existence! First of the month she was told he had congestion and other issues so would be put on antibiotic. 2/13 she was told Antibiotics were started. He still wasn't feeling well so in the 21st she was told they were beginning antibiotics. She said she was told they'd already began administration. They then told her there was a delay with Dr. and lab!!! They are not accredited by JCAHO so we will file complaints with Medicare/Medicaid and Illinois Droartment of Health, Gary's response to her today... you can take him somewhere else if you're not happy. Stay away if possible!

I went to see my husband yesterday he looked like he had been in the woods for two weeks, hair dirty room dirty, someone else’s glasses on that are scratched up and bit his apparently got lost and they found these and decided they were his. They’re not they’re black or bronze. All of his shoes are gone all but one piece of clothing of his is gone his hair trimmer was gone and he has a huge scrape on his left arm, which he doesn’t have use of because he had a stroke 10 years ago and he can’t move that arm at all so apparently they left it dangling while they went through the shower door and scraped his arm and said nothing about it to anybody. He also has a bed sore.

I recommend staying away from this facility, especially a patient in need of hospice care. We moved our nonverbal mother here and there was little communication between the facility and hospice provider(Traditions). Both shifted blame to each other on various things. We made sure someone was always in the room during the day as there was little checking on our mother by the nurses. We would get an occasional head popping in the door asking if we needed anything, but not a regular check on our mother. We raised these concerns to an advocate multiple times, but nothing changed. A supervisor came and indicated that someone should be coming in every 2 hrs. We explained that was not happening. At one point during our visit, a patient across the hall was yelling for help. After 3-5 minutes, I told him that I will find someone for him. I tracked down a nurse to provide him with assistance. The biggest issue, the day nurse LIED about when she was administrating pain meds. In the hospital, meds were scanned so they know when meds are given and what doses. This is not done in this facility. We were told they write information in a notebook, which provides no proof when meds are given as it is easy to falsify. Our hospice nurse admitted that she was not confident that meds were given when they should and she was going to start tracking the amount of meds that is still left in the bottle so she could gauge how much was given during the day. On my mother’s final day, facility nurse came in at 10:30AM to give a dose of pain meds. We didn’t see another nurse until the hospice nurse showed up a little after 3 PM (Hospice Nurse is supposed to come twice a day). Hospice nurse asked when the last dose of pain meds was, which we shared was 10:30 AM. She said that our mother needed pain meds, and she can tell that she’s in pain by her breathing. Facility nurse came in at 4:10PM to provide pain meds. Hospice nurse said she is to start receiving pain meds every 2 hours. At 6:15 PM, we go to the nurse’s station to ask for the pain meds as it has now been 2 hours. We were told that she got her pain meds at 5PM. We shared that she didn’t get anything at 5PM as we have been in the room the entire time and nobody has come in. They looked through their paper book of records, and said “Oh, you’re right”. They came in at 6:20 to provide the pain meds. We shared our continued concerns with the night nurse, and she told us that we should have asked for pain meds if our mother was in pain. We asked how we are supposed to tell if she is in pain when she can’t speak and it should be the job of the staff to help determine this. The night nurse also called the day shift nurse, while in our room and on speaker, questioning about the time of the meds. The day shift nurse continued to say she gave the meds at 5PM, which we shared was not true. (She wasn’t due to have meds at 5 as she had just gotten them at 4:10). We told the day shift nurse she was lying as she didn’t give meds as we were in the room the entire time and she became loud and combative, and threatened that she was coming back to the facility to confront us. I asked the night nurse to take the phone out of the room as we were done with this conversation. I also asked for an administrator multiple times and was never given one. Our mother passed at 8:40PM. The facility called the hospice nurse to inform them of the passing and they indicated that they were over an hour away and didn’t arrive to the facility until a little before 11PM. Hospice was supposed to be coming twice a day, so it was apparent that she hadn’t planned to come back for her second visit had our mother not passed. I talked to the corporate office, and they indicated there have been some recent changes with admins at this location. Not sure what an admin can do when they have staff who just doesn’t care about their work or the care of their patients. I encourage anyone who is reading this post to look back through the reviews as this is a trend.
