This place is the best from the CNAs to the the Nurses. The trach unit personnel works so hard and I’ve never seen a better team.

Greenwood Healthcare provides great care and has knowledgeable, compassionate staff. When you see longevity in the staff, that is your first clue that the staff are there for the right reasons. You cannot go wrong with Greenwood Healthcare center for rehab, memory care, and long term care needs!

Do not send your loved one here. Their life could depend on it. Read as many reviews you can find, read Board of Health reports, Medicare/Medicaid reviews, and look for all news stories about the facility and the companies that operate them. I feel guilty and am mad at myself for putting my loved one in a situation where he feared for his life. We chose this facility because there are very few places in Indiana that take tracheotomy patients that are on ventilators. It was also closest to our home. Luckily my loved one and I were able to get him out of there in a little over a week. I called his caseworker to complain of a few easy things. Like 30 minute call light wait times for a VENTILATOR patient. I told her that he hadn’t been bathed, had a gown change, or sheets changed. He got a gown and sheet change but no bath. If a facility struggles with basic human care then what other areas are they struggling with? I must also mention that my husband is an amputee and could not get up on his own. The following are the rest of our complaints: My loved one contracted me because the call light was not being answered. His roommate (vent patient) fell out of bed and was on the floor. I had to call the front desk to call the nurses so they could provide help. Took several minutes for them to respond. No bed rails or bed alarms to keep this patient safe. Call lights taking 30 minutes or longer. Bug bites in husbands arms. PT and Ot were basically non existent. Super small double occupancy rooms with no privacy. If the curtain was open my loved had to use his urinal in front of his roommate, in front of staff/residents/visitors walking past his room.Oxygen generators so loud that you could not hear normal speech and loved one unable to sleep because of it. My adult son and I both witnessed each time we were there that the staff would be scrolling or talking on their phones. Hanging out at the nurses station chitchatting. Just generally not caring to do their jobs or do them well. While at previous facility had several days of being on Airvo for 12 hours. Loved one only on Airvo at this facility for a total of 12 hours for the 8 or 9 days that he was there. Staff not always wearing gloves while providing trach care. I observed this for myself more than once while on FaceTime with loved one. After waiting the typical 25-30 minute call light time normally a tech would respond. If my loved one needed suction he would have to wait an additional 15-20 minutes for a respiratory therapist to respond. In an emergency for a ventilator patient these times are unacceptable! My loved one would call me having high anxiety, panic attacks, crying, trembling, and telling me he was going to die if he stayed there. He probably would have too! I told his nurses and techs several times that he had a groin rash. Completely ignored. He had to use a urinal and it had to sit on the tray table that he also ate off of. His food was often served beside the full urinal. He couldn’t reach the floor so that was the only place to put it. Told staff that he had severe pain in his side and that it also hurt to pee. They wanted to place a catheter in his bladder and give him an enema. He told them no that he wanted to go to the ER. At first they told him he couldn’t go. They finally relented at sent him to the ER. Once arriving at the ER he told every staff member that provided him that he feared for his life at Greenwood Healthcare. Thank goodness Indiana has a law that an ER. Cannot return you to a facility or released to a person that you feared for your safety. My loved one spent over a week in the ICU after leaving Greenwood Healthcare. There is no excuse for the nightmare that this facility put my loved one through. Stray dogs are cared for better!

Our experience with Greenwood Healthcare Center was unacceptable and deeply concerning. During my father’s stay, my family and I personally witnessed repeated issues that raised serious concerns about the quality and consistency of his care. These concerns were brought to staff at the time, yet nothing changed. As his daughter, I had to constantly advocate for him and raise concerns simply to ensure his basic needs were being addressed. Despite this, issues continued. The situation placed an extreme amount of stress on my family and on my dad during an already difficult and vulnerable time. My father has since passed. I am sharing this so others are aware and can make informed decisions. Families should not have to fight this hard for appropriate care. Transparency, accountability, and dignity in care truly matter

Greenwood Healthcare Center truly stands out because of its people. The longevity of the staff and management speaks volumes—many team members have been here 20+ years, which is rare in healthcare and says everything about the culture and leadership. That kind of consistency creates trust, strong relationships, and continuity of care for the residents. While the building itself is older, what matters most is what happens inside it. The staff is compassionate, dedicated, and genuinely invested in the residents who live there. They provide excellent care and treat residents with dignity, respect, and kindness every day. The heart of this facility is its people, and that’s what makes Greenwood Healthcare Center a special place.
