Creekside Health & Rehabilitation Center has a welcoming and caring atmosphere. The staff are compassionate, attentive, and dedicated to providing quality care to residents. Team members work hard to ensure residents are treated with dignity and respect while maintaining a clean, safe, and comfortable environment. Communication is handled professionally, and there is a strong focus on meeting the needs of both residents and their families. It is evident that the staff genuinely care about the well-being of those they serve. Overall, It is a facility that strives to provide excellent care and support, and the commitment of the team is truly appreciated.

I recently visited my grandma here and just wanted to say how amazing the unit manager, Kelsey, and her team have been. Everyone was so kind, welcoming, and patient, and you can tell they really care about the residents. My grandma seemed happy, comfortable, and well cared for, which means a lot to our family. The staff does a great job making sure everything runs smoothly and always takes time to check in with families too. It’s such a relief knowing my grandma is in a place where people genuinely treat her with kindness and respect. Thank you all so very much for everything you do! ❤️

We are having a truly terrible experience with the care my father is receiving at this facility. Basic standards of dignity and hygiene are not being met. He went an entire week without a shower, and when he was finally bathed, he was placed back into dirty clothes. On another visit, he was found lying on top of hangers underneath him, which is both uncomfortable and unacceptable. Most recently, my mother arrived to find him sitting in a bare bed without proper bedding. Communication is equally concerning. Attempting to reach someone by phone takes hours, if you can reach anyone at all. There are no clear or accessible contact numbers on the website to speak with someone in charge, making it extremely difficult to address urgent concerns. Families trust facilities like this to provide safe, respectful, and attentive care. Unfortunately, our experience has fallen far short of these expectations. I strongly urge management to address these issues immediately. I’m sure like other reviews they will ask for me to reach out- to the same number u can’t get a hold of anyone at.

I cannot say enough great things about Creekside Health & Rehab! From the moment you walk in, you can feel the positive and welcoming atmosphere. The staff is compassionate, attentive, and truly dedicated to providing quality care to every resident. The nursing team is knowledgeable and responsive, always taking the time to ensure residents and families feel informed and supported. Therapy services are outstanding, helping residents regain strength and independence with encouragement every step of the way. Since the new Administrator, Samantha Proctor, has come on board, there has been a noticeable improvement in overall operations, communication, and resident satisfaction. Her leadership is clearly making a positive impact, and it shows in the team’s morale and the care being delivered. I would highly recommend Creekside Health & Rehab to anyone looking for a facility that genuinely cares about its residents and strives for excellence every day!

Thank you for the opportunity to have some way to express my concerns. I have tried to setup care plan meetings once we did and...guess what, no one showed up, the staff tried to get something thrown together quickly with no thought of my time, or how many appointments I had to reschedule to attend the scheduled meeting. Staff is so very, very unorganized, not to mention the total LACK of communication in each and every area. They can't get organized to set up another care meeting! I've been waiting months. My other concerns are, the smell of urine....the stinch can't be escaped you breath it in the entire time your there, the lack of cleanliness, sticky floors everywhere you walk, the hands of my brother are always dirty, nails full of old food, hair that his greasy and smelly, his clothes we purchased and shoes, slippers, blankets all have disappeared! I purchased new sweats and slippers for him to go to his surgery appointment and even gave them to the nurse, who put a note on the bag, put it in your computer to please don't send him off to surgery without a shower, clean hair, clean clothes and a shave an hair cut, to avoid infection. Did staff do any of it???? No they did not. I've been very clear to everyone there that this care is inhumane, sad, heartbreaking frankly unacceptable to any human muchless a senior who has worked all their lives and in their last days they are treated as something disposable, invisible, not worthy of the same care they themselves would do, for themselves before becoming sick. Families shy away from the memory care unit, because of the lack of care and such a sad, depressing state families have to watch their loved one drift away. Tucked way back in the corner of the facility...who's watching?
