My 93 yr old mom entered the Coquina facility from a severe fall with a fractured spine, fractured right pelvis, along w being incontinent. Within 60 days at the Coquina facility, she is being released. She is walking on her, going to the bathroom on her own, dressing on her own. She was treated with respect from the PT, OT, Nursing & Aid staff. I’m addition, the Activity staff was amazing. They arranged for some wonderfully talented musicians entertaining the residents with lively interactive music, which my mom loved. I’m grateful to the Admissions staff providing an honest and thorough review of the facility. Also, the Administrative staff, was very helpful in addressing any concerns or issues through our meetings or through email communication, which was extremely helpful. Finally, I want to thank the Maintenance staff. On my daily visits, I was very happy to see the cleanliness of her room and the entire facility. This may not be the most beautiful place, but how your loved one is treated, takes precedence over everything. Thank you to everyone at Coquina for helping my mom get to come back home. You will always be remembered. We plan to come back and visit! ❤️

It was first time visiting my sister. She seemed very satisfied with her surroundings. From my perspective she was being cared for well. Staff was very attentive and very helpful with my questions.

A friend of mine admitted herself in Coquina a bit ago. She is facing the last couple stages of Parkinson’s and has bipolar. She had a fall last year and ended up in Coquina for her broken leg. Physical therapy and she thrived while being there. Upon her release, within a week you could see she was confused, lonely and forgetting to take some of her medications. The “light” she came home with was gone. After another fall and she decided it was time. I can say the staff ( cna’s, rn’s, dietary, physical therapist, activities and laundry) are great. I’m grateful she is happy. It is where she wants to be, that on own should speak for itself.

Hi, I recently had a friend admit there. I have to say the staff is friendly and it is very clean. My friend's goal is to return home and the therapy is excellent. She is making great progress and should be able to discharge home in about a week. I am pleasantly surprised, as I previously did not think well of Skilled Nursing Facilities and this Facility has changed my mind. If I ever need rehab, I will choose this facility! I highly recommend it!

I did not stay at this place, but a loved one of mine did. This place is dirty. The administrator is heartless. As for the staff, most of the nurses are amazing, they work hard and care. Only problem is there's not enough of them. One evening I went to bring my loved one dinner, and a nurse was in there changing her incontinence underwear. My loved one had such a bad rash in her private region she asked for my hand to grab on to due to the paint of being wiped. From this point on i made sure to change her, cleaned her and powder her every morning. The floor in this establishment had ants crawling all over. Even right after her room was cleaned we went in and food was still on the floor with ants all over it. I had to change a bandage that was covering a wound. This bandage was left on so long the smell was so bad it discussed me. The bathroom was left without soap for over two weeks, would have been longer but patient was released. The only good thing I can say about this place is they have an amazon physical therapy department. Unfortunately I was unable to upload a picture of the chip w/ants and the bandage to this post. P.S. When I took the picture of the chip a nurse accused me of planting it there, and quickly called the administrator to report it.
