LOST MY FATHER AT THIS PLACE YESTERDAY! EVERYTHING HAS BEEN DISORGANIZED AND I WASN'T INFORMED OF INCIDENT/DEATH DIRECTLY. HE WAS A YOUNG MEN AND DEATH WAS UNEXPECTED. HE WAS DOING GREAT THESE LAST SEVERAL MONTHS.

This has to be the worst ran facility in the city of Chicago. If you wanna see your love one alive again, Do Not Go There. There’s no care Team, nobody cares if you ask questions everybody gets offended as if you are testing the knowledge of their skills, I was always told if you don’t know just say I’ll find someone who does, I’ve been in the health field for 35 years, so I’m very patient with healthcare employees, but when everyday the facility smells of weed and they’re letting Diabetics sleep through breakfast and lunch saying that’s they’re right and now they’re getting sent to the hospital with a blood sugar of 20 that’s not right. 1 week later in bed again had a stroke was sent out and never opened their eyes again, never had his oxygen on, the staffed checked and changed him at their convenience gave medication when they wanted to not according to the order. Talking about Abuse and Neglect, here it is! This Hell hole is still open because people are complaining among themselves, not me I’m going all the way up to the top of the Ladder and make sure I’m heard

This facility has extremely unsafe conditions. There are ongoing issues with bedbugs, rats, and roaches. Patients have reported serious abuse, drug use, and other harmful situations that were not taken seriously or properly investigated. Staff frequently fight, and my loved one was treated badly and often left without clean clothes or proper care. This place urgently needs to be investigated.

This place is horrible. The cna are rude and have an attitude as if its a hassle to do thier job. An absolute embarrasement. Its takes hours to get my mom changed. This is the 4th nursing home my mom has been in and its the worse ive ever seen. My advise to this nursing home and staff is to have empathy for these older sick people. How would you feel laying in waste for hours? Rooms an floors are filthy.
