My father has been at avir for about 3 years and he's been treated with respect and his cnas are great my father likes it there and also his nurse mrs.minchico is so awesome he loves her and I think this facility is great also the whole staffing is great I've worked at these places nothing like avir.I would totally refer your loved ones to choose avir nursing rehabilitation center.ty to all of you.

NEGLECTFUL!!!<br>My review is regarding the care of an 83 year old, incontenent, disabled, visually impaired woman who needs a wheelchair. She has spoken to me a few times regarding her care and despite reaching out to them for her several times, she continues to receive inadequate care. Below are the concerning things happening at this facility....<br>failure to help from bed to wheelchair on multiple occasions<br>(she is working on gaining strength and mobility and cannot stay in bed all day)<br><br>failure to change her pads/diapers in a timely manner leaving her sitting in a wet bed and clothing for hours<br><br>failure to administer pain medication on time, causing her extreme pain<br>They close her door despite her asking them not to so that they don't have to hear her crying in pain<br>rude and dismissive staff (just yesterday a nurse threatened her, stating if she didn't shut up she was going to recommend she go to a psych hospital. (She was crying out in pain from the same nurse not providing pain medication...made her wait over an hour after the time she was supposed to take it, despite her cries of pain)<br>failure to assist her setting up her ventilator or requesting manufacturer help for over a month now (she was hospitalized a few months ago with dangerous levels of Co2 in her blood and needs this machine)<br>failure to disclose what medication she is receiving when it is administered despite asking<br>failure to administer eye drop medication needed to help her vision stating prescription issues but nothing is done to correct<br><br>I have reported this facility...DO NOT send your loved one here!!!!! Begging you...

My family had a very disappointing experience with this facility. Over time, the quality of care has declined significantly.<br>My grandmother passed away in January after developing double pneumonia while in their care. For about a week, we repeatedly asked staff to have her properly evaluated and sent to the hospital, but our concerns were not taken seriously.<br>When we visited her, she was clearly unwell, yet we were told she would just be monitored. The next day, we received a call from a staff member—not her nurse—asking which hospital to send her to, and they were surprised we hadn’t already been contacted.<br>When she arrived at the hospital, no medical information had been sent with her, and we later learned she had coded before transport without our knowledge. By then, her lungs were filled with fluid and she was struggling to breathe. She passed away shortly after.<br>We believe earlier intervention and better communication could have made a difference. This was a heartbreaking and preventable situation.
