I’m going to try to be fair-the staff are good people in a heavily neglected and less than prepared environment. The facility itself is in dire need of repairs and updating. I did not see much or any sanitation practices. My wife’s trach suction catheter was kept wrapped up in a dipper. The rooms are hot even with ac units, a good environment for potential infection. My wife is in a fragile state and this facility was not prepared or capable of her high needs. They should not have accepted her in my view. We were there for 6 days and I planned on having her transferred home in the next day or two, however, she most likely contracted pneumonia at Award Care and we have been fighting it in the ER and ICU. She is on track to recovery but she will not return there under any circumstances. I wish anyone going there the best and if you can leave, do so asap. I only gave them 2 starts because some of the PCAs and LPNs actually cared about her but they were too ill prepared for me to trust them in any sense.

Here for rehab and have no issues with the place. Caring staff, excellent therapy program, good nourishing food. Consider the difficult job they have and the little inconveniences that you may have are nothing. May GOD bless their work.

Follow up from the owner’s suggestion: I have reached back out to Sarah Pena on several occasions regarding my property in getting this resolved and she still have failed to return any of my calls or reply to any of my emails. I’ve attached pictures of the damaged that was done to my pool screen and how my pool currently looks as a result l. In need this issue resolved ASAP !! This facility tree fell down on my pool screen that has caused extensive damage in which they haven’t yet sent me the cost to repair it. My deck and my pool is suffering damages due to the debris from their trees leaves and sticks falling on the deck that causing stains and causing clogging issues to the pool. I recently had to pay to have my pool repaired because it was clogged up with sticks and leaves. My deck will need to be pressure washed to try and get rid of the stains as well. As I’ve expressed to Sarah this is my home and this damage is causing my value to decrease. I’ve lost all communication with Director Sarah Pena who refuses to let me know whats going on with getting this repair done as it has been over 3months since the accident occurred. very upset home owner Mrs. Smith

During the final 21 days of my cousin’s life—who was more like a brother to me and whom I had the honor of serving as caretaker—I visited him daily at Harts Harbor Rehabilitative/Nursing Home Center while he received care and rehabilitative services. He had been diagnosed with terminal cancer that had metastasized to his bones. Our initial hope was that rehabilitation would help him regain some physical strength and independence so that he could return home. Sadly, he transitioned on February 4, 2026. Although our outcome was not what we prayed for, I cannot express enough gratitude for the exceptional care, compassion, and professionalism demonstrated by the staff at Harts Harbor. Every question our family asked was answered thoroughly and respectfully, with clear knowledge of his diagnosis and treatment needs. The staff consistently ensured his comfort, dignity, and overall well-being. The East Wing team, under the leadership of Ms. Kawana G., was truly exemplary. Ms. Kawana, the Unit Nurse, demonstrated honesty, compassion, clinical expertise, and attentive patient-centered care. Her professionalism and kindness provided reassurance to our family during an incredibly difficult time. Our family will forever be grateful for the outstanding care and administrative support provided by Harts Harbor Rehabilitative/Nursing Home Center. The level of dedication and heartfelt service shown to our loved one will not be forgotten. Harts Harbor is clearly committed to providing quality care, and I encourage continued efforts toward growth and improvement that will further advance patient comfort, clinical excellence, and staff support. Ongoing investment in both patient well-being and employee development will only strengthen the outstanding foundation already in place. We look forward to returning to express our appreciation personally and to extend gifts of gratitude to the remarkable staff who cared for him so well. With sincere appreciation, Melton Family

I would leave less than a half as a star if I could this place was supposed to take care of me and said they're Wound Care Unit let me turn to gangrene they refused medical service would not let me call 911 and refused to call a doctor and to see me immediately when it started to smell. Once a doctor was brought in to see me he sent me immediately to the emergency room where are they told me upon seeing me that I was definitely going to lose my left foot possibly my left leg they would try to save my right leg to which no avail they were able to I lost both legs I filed lawsuit to which they filed bankruptcy I am not the only one that filed laws filed a lawsuit three others did so as well and we are left in a medical situation and to which we cannot afford to be taken care of appropriately and no longer trust a facility to do so the medical staff there has failed along with the employees there janitorial staff cares more about the residents Tony is the best there and does not get the recognition that she deserves neither does Matt the only nurse there that cared
