DO NOT leave your loved one there! The staff is TERRIBLE leaving my mother in a soiled bed for HOURS before finally getting her cleaned up. They never come when you ring the bell either. We had many meetings with the higher ups in there and it was fine for a day or 2 but then went right back to the way it was. They're SO unorganized it's not even funny. Please if you care for your loved one DO NOT put them in there for ANY amount of time. The food comes cold, they don't clean the rooms often. There's 1 aide that's the worst. Never got her name but she's African American with either a pink bob wig or a long pony tail wig. That one needs to be fired IMMEDIATELY!

Admission triage RN is very nice on the phone and acts like they all have it together but when you get there the place is run down, needs major updating and they have nothing ready for your arrival. Patients bells ringing forever with no one going in for hours. RN doing wound care was rude, inattentive at admission and did not know what she was doing but also didn’t have any help. On top of that she first brought another pts medicine and gave it to our family member, patient told her this is not mine over and over so she had to leave and go figure out her mistake. We waited 3.5 hours with a leaking wound for wound vac to be applied incorrectly. They didn’t have patients medicine on admission and suggested they would have it the next morning even though I was told the day before it was all ordered. We called 911 and got our family member out of there. Paramedics said RN was very rude to them on arrival. No one has called since to even check in or ask anything.

My mother Connie is at Autumn Lake for short term rehab. She has PT and OT 5 days a week. Her favorite aide is Kyle. He is very kind, caring and attentive. He has made her stay bearable. She looks forward to seeing him.

I was truly happy with my experience. The caring/nurse staff, physical therapy staff, front desk and the food was great. I was able to relax with knowing my husband was well taking care of. I recommend this facility as first class.

My family member stayed at Autumn Lake Healthcare at Madison, and we had a very good experience from start to finish. The staff was kind, helpful, and always willing to answer our questions. The care was attentive, the atmosphere was warm, and the facility was clean and well-maintained. We felt comfortable and confident knowing our loved one was in good hands. I would definitely recommend this facility to others.
