Rehab only. DO NOT BRING ANYONE FOR LONG TERM CARE OR MEMROY CARE. This facility falsely identified as memory care with long-term wing and rehab wing which is a complete lie. Brought my family member there with severe dementia, explained the care they needed, the staff said they were equipped to handle and within 4 weeks it was very clear they were not. It appears they were only thinking of the pay check of selling a bed. Absolutely disgusting. The lies didnt just happen during intake, it continued the entire stay, when asking nurse staff questions the only answer recieved was "I dont know",or"I didn't work that shift" no one ever bothered to communicate or check notes or patient file. They had a revolvong door or rehab patients as roommates. Staff had no awareness of adverse effects of medication to my family member, when medication was added or adjusted they did not monitor for risks, which at point almost caused a medical emergency. Ignores clear signs up psychotic, hallucinigeic episode as if they are not happening, and even denies they are happening even when a discussion is had. Took full code status from a cognitively absent individual even when documentation presented stated otherwise. These are only a few things, there are 100s of incidents. DO NOT RECOMMEND IF YOU CARE ABOUT YOUR PERSON(S).

My Mom has been to Applewood several times for physical therapy & they’ve been amazing. The staff are very helpful & friendly. I feel it’s one of the best nursing homes around. A wonderful facility.

My mom had rehabs stays at many facilities in southwest NH, Applewood was by far her favorite and the most kind! They cared about her, listened to her, met her requests as able. The care they provided her in her final years was outstanding. They staff was amazing in assisting me to complete the complicated paperwork from DHS, after years of other agencies pointing me to dead ends. I cannot say enough good things about them. The best elder care facility in the region!

The staff at Applewood are totally amazing! I cannot say enough good things about Applewood! My Mom stayed there for five years, and has been gone for five years today also. My dear friend past today. She was transferred on Wednesday to spend her remaining days. Once again the staff was awesome! Thank you

If you care for your loved ones, don’t leave them at Applewood. The facility isn’t as clean as you would expect it to be, patients eat in the hallways, rooms are small and uncomfortable, if you want a TV in your room it will be 2 feet from the bed. Don’t let them do the laundry for your family member either, they lose clothes, I had to keep replacing them. My father was there after a hospital stay and then developed pneumonia and the flu, he was too week for PT but they just turned that into him not being cooperative. At one point they told me he was depressed and recommended Hospice!!! He was 96 years old and had just lost his wife of 66 years. We finally moved him to a new facility and he is doing wonderful, the facility is clean, the staff is exceptional, they eat in a dining room and there are all kinds of activities and if you can’t get to the activity room they come and get you, they go outside when it’s warm and he participates in the daily activities…….the person they recommend for hospice! My father is a new man since he left Applewood and is happy being a resident where he is now. And the exceptional care he is receiving is costing us less than it did at Applewood.
