This statement is provided to formally document serious concerns regarding the care and treatment my mother received while residing at this facility. Our decision to place her there was based, in part, on the apparent absence of prior abuse citations, which we reasonably interpreted as an indication of acceptable standards of care. Unfortunately, our experience demonstrated otherwise. During her short residency, my mother sustained significant and unexplained physical injuries, including extensive bruising that has persisted for multiple weeks. At no time were we notified of these injuries, nor were incident reports or required documentation provided. Despite direct requests, including those made to the facility’s director no records were produced. This lack of reporting and transparency raises substantial concerns regarding compliance with mandated reporting requirements and patient safety protocols. Additionally, interactions with facility leadership were unprofessional and retaliatory in nature. Following a negative review submitted by a family member, I was subjected to being called pathetic for begging them to take my mother by the director. The removal of that review further raises concerns regarding suppression of legitimate complaints. The care provided to my mother reflected a pattern of neglect. She was frequently left unattended for extended periods despite requiring continuous supervision and medical support. Physician-ordered equipment, including a CPAP machine, was not consistently utilized. Requests for reasonable safety accommodations, such as bed rails, were denied even after repeated falls. Although the facility indicated that alternative safety measures and one-on-one supervision would be implemented, these measures were not reliably carried out. Remote monitoring consistently showed my mother alone during times when supervision had been explicitly represented as being in place. My mother was admitted following two strokes and required structured rehabilitative care. Instead of improvement, her condition deteriorated during her approximately two-week stay. Upon transfer to another facility, her condition stabilized and began to improve, further underscoring concerns regarding the adequacy of care she previously received. As a result of these events, formal complaints have been filed with Medicare and Adult Protective Services, and additional actions remain under consideration. The circumstances described above raise serious concerns regarding potential neglect, failure to follow physician orders, inadequate supervision, and possible violations of patient care standards and regulatory requirements. This matter warrants thorough investigation to ensure accountability and to prevent similar harm to other patients. How nice of you to claim that it was undeliverable, I gave you my email you have my phone number yet still no reports. I was unaware that requesting safety measures that were in her chart were demands or requesting that my mother be showered on a regular basis was so taxing on you all. I would recommend to anyone who actually cares about their loved ones DO NOT place them here!!!
