I am writing to formally express serious concerns regarding the care and conditions experienced by residents at this facility. My father is a resident, and I am deeply troubled by what appears to be neglect of basic responsibilities owed to residents and their families. Families trust these establishments to provide safe, dignified, and appropriate care in exchange for the services and payments being made. There are ongoing concerns regarding the cleanliness and overall condition of the facility, including strong odors that suggest inadequate hygiene and maintenance. These issues raise serious questions about whether proper standards of care are being consistently upheld. Elderly and vulnerable individuals deserve respect, dignity, and a safe living environment. They should never be subjected to conditions that fall below acceptable standards of care. Additionally, staff attentiveness and responsiveness are critical. Concerns that issues are being ignored or not properly addressed must be taken seriously and corrected immediately. I am requesting a thorough review of the facility’s conditions and care practices, along with prompt corrective action where necessary. Residents and their families deserve transparency, accountability, and meaningful improvement. 6.26.2026 I am deeply concerned about the care my father is receiving at your facility. During a recent conversation with him, I personally overheard him request that his bedside table be moved within his reach because he is physically incapable of doing so himself. I then heard a staff member refuse this simple request, despite the minimal effort required to assist him. I documented this interaction because it raised serious concerns about whether my father's basic needs are being met. Throughout the same day, I heard my father repeatedly request ice, yet his requests went unanswered. He later told me that his roommate had also repeatedly requested coffee throughout the day without receiving assistance. While I cannot independently verify his roommate's experience, I can verify that I personally heard my father's repeated requests for assistance and witnessed his increasing frustration as those requests were ignored. My father is an incapacitated individual who relies entirely on staff to meet his daily needs. When essential items are placed out of his reach or assistance is delayed or denied, he has no ability to resolve these issues independently. These circumstances not only compromise his comfort but also undermine his dignity and quality of life. I have also been informed that his wheelchair and other necessary items have been placed out of his reach, further limiting his independence. If this is occurring, it is unacceptable for a resident who depends on staff for mobility and basic care. I recognize that my father can become frustrated and verbally expressive. However, based on what I personally observed, it appears that his behavior may be, at least in part, a response to repeated unmet needs and prolonged delays in receiving basic assistance. Ignoring requests for simple necessities can contribute to unnecessary distress for any resident, particularly one who is seriously ill and unable to care for himself. Because I live approximately 1,300 miles away, my ability to intervene directly is limited. Nevertheless, I have carefully documented what I personally heard and observed. I have also informed my daughter, who lives nearby, and we intend to follow up regarding these concerns. I will be requesting a conversation with the facility's owner and executive director to discuss these issues and to ensure my father receives the compassionate, respectful, and timely care that every resident deserves. I will be respectfully requesting that these concerns be investigated promptly and that appropriate corrective action be taken to ensure my father's needs are consistently met. God help my dad and help me deal with and help him properly!!! Jesus s take the wheel!!!

I am here currently. While it is an older facility it is kept clean. Staff are always around if anything is needed and are quick to help. Have been here 2 months with no complaints. Between the rehab team, nurses, cnas, admin, housekeeping and everything in between the staff has made me feel truly cared for. There are definitely some special people working here who go above and beyond to make sure residents are taken care of and comfortable.

This place is horrible. The facilities are nasty. The food was horrible. I had to move my mother to another facility. Employees on the night shift are loud and unprofessional. The place has a foul odor. "Bianca" was horrible to deal with. She was absolutely no help. Luckily they had an employee step up and help get my mother processed out of there. I feel sorry for the poor people that are stuck there and can't get out.

Horrible facility. My grandmother had to do rehab here and there is hardly any house cleaning done. Her first day here when I went to see her I had to clean her bathroom up. The toilet was disgusting. And there was a used urine collection hat on top of the toilet. And the nurse tried to lie when I first got there the first day about " talking to my grandma" about leaving her medicine unattended on her table and not telling her what it was. Then for thanksgiving she wouldn't send her out to the hospital to take her PICC line out for the day and tried telling her they don't send out for the hospital. And then they're said they could take it out. Uhhh no you don't take a picc line out in a facility. Especially that facility. And I was on the phone with her and a random patient kept coming in the room and wouldn't t leave and no one at the nurses station answers the call bells. And she was even screaming for someone to come get him out of the room. I then tried calling up there to get them to go check on her and they let the phone just ring and then after a few time of trying to call of up there they started hanging up each time. So... Don't send you're family or recommend friends here. It's a crappy place. I ended up looking into another rehab for future purposes. I learned to do review research on these facilities now.

The staff at this facility are all so kind and caring and it starts at the top! Administrator is actively involved with his residents and their family’s as well as the staff. The director of nursing and team work tirelessly to make sure everyone is well cared for. Therapy is top notch! If you or your family find yourselves needing a facility, I highly recommend touring and meeting them and seeing for yourself!
