The CNA's do almost everything. There are people living there who don't even know what day, month or year it is. God love 'em. My mother has her wits about her, but can't come home just yet. She was being charged $8,000 a month & now they are charging her $3000.00. She is getting food that is served in a styrofoam container (a to go box) & the food is cold. Today she was served lunch with plastic wrap over it. It was dated 12/5 & today is 12/8. I'm not quite sure what is going on, but if she's paying that much to be there she should at least have HOT food AND the things that she ordered. Not a cold cheeseburger, with a side of cold mashed potatoes. She has been served in a styrofoam container many times & even on a paper plate. What in the hell is going on that would make the facility think that it's perfectly fine for them to do? This is supposed to be one of the best places in Indiana, so I can't even think about the worst! Someone needs to be held responsible, instead of the office being told about it ~ and "looking into it!" Obviously nothing is being done to change it, but they'll still take the $3,000 a month. Somebody is lining up their pockets & don't even care what's going on. I hate it for the people who have to be there, 1. bc they have to be until they are healed (rehab part) but it's still just a different hallway 2. bc they have no idea who they are or what their name is 3. bc their only way of getting the care they need is to be there 4. this is the place they are going to be until they die. That is awful!!!!!

Bad experience and our grandmother had a stroke and couldn’t speak but a few words. Her last final days were her very scared that the nurses there were saying bad things to her and being mean. When she was feeling helpless wouldn’t recommend

My mother just passed away in May, 2022, She had stopped eating, I had asked the nurse practitioners how long can my Mom go with out eating? They told me she is eating a little off and on she should be ok. They did start her on a depression pill that was to give her an appetite. They told me to give it two weeks to start working. I had to ask two times to take her blood to see what that would show. Then I had to tell them that she needed a chest x-ray to see if she had pneumonia. She did in deed had pneumonia. She gotgot so weak I finally took her to hospital.Her urine was a murky brown, she had so much infection in her, blood in urine, blood in stools and so on. Do please watch your love ones yourself and make sure you take them out of facility to a doctor. Do not rely on the doctor there

Unfortunately, my mother was placed here after a two week stay at IU West. It was my mistake. I toured three rehab facilities in this area. Villages of Avon put on a great sales pitch. All sounded and looked great. That wasnt the case. The majority of the staff is uninterested and abrupt. Understaffing is obvious. Once you pass thru the “double doors” the mood and smell drastically changes. My mother arrived wheel chair bound (physically fine before two weeks in hosp). She was only in rehab for PT, OT and get rid of a catheter. These people had no interest in getting my mom rehabilitated. Her care was subpar. Here are examples: 1. nurses call light on for three hours to ask for asst to bathroom. No one comes and my mother lies in feces for two hours. This happened twice. Once, I called nurses station to demand the nurse respond to her call light. They react fast when the family knows what is happening. 2. They had absolutely no interest in helping my mother’s cath come out. I meet my mom at a gyn appt and her cath is full of dark dark red blood! Not kidding! When I questioned the head nurse, “we usually wait until the urine is darker than that before calling our dr”. Gynecologist was appalled. I took her straight to ER. ER dr and staff are appalled. Villages of Avon were unsure why I was upset! Villages of Avon did not miss my mom for 4 1/2 hours when she didn't return from gyn appt . Called me at 4:50 because driver was leaving at 5:00. While she was in ER, I made phone calls and met with a superior facility that was out of network, but there was no choice. Villages of Avon’s staff were taking her down death row. The head of PT is full of bull, but talks a good game. She needs replaced. Nurse Tara = lazy and appears under trained. Nurse Aide John = night shift is the only shift they can hide him. His appearance is not professional and getting help from him req’d several requests. He was so busy sitting at a cubby in the hall with his back to everyone. His pants rolled up to his knees with paper clamps holding them up (no lie). I abruptly removed my mother from this below acceptable treatment and rehab facility. The lead nurse was more than put out by checking my mom out. So unhappy, that an entire page of her meds was not sent to new rehab center. Just DONT go to Villages of Avon. Don’t put anyone you love in that subpar facility. I have also reported to the state. I want to point out two shining stars that do exist within that smelly, terrible food, lazy rehab facility. Joseph and Katie Ann. Wonderful hearts and compassion. This facility actually told me that I needed to realize that my mom would be in a wheelchair with a cath for the remainder of her life! I’m very pleased to report after 2 1/2 weeks in a superior rehab facility, that was totally engaged in her health and outlook, my mother is freely walking with no cath and living independently in a beautiful sr community. One more thing, Villages of Avon wanted to diagnose my mom with dementia. That was their easy way out. Then no one would believe what she was saying. An elderly person arrives at a rehab facility under stress and uncertainty of their future independence. Its scary!! Of course, they are going to ask the same questions everyday. Who wouldn’t?? They are also lonely. The staff has no time to spend with them or show compassion. Just label them with dementia and let them lay in feces for two hours on multiple occasions. That’s a humane way to help patients recover!!! There are better options for rehab than this place. If you choose it - you were warned. I wish I could give zero stars. My one star is for the social worker. She was helpful the last few days of our miserable experience.

Do NOT put your loved one in this facility!!! My grandmother was over medicated to the point of near sedation and the LPN on staff refused to get us her for her. We had to call an ambulance to take her ot the ER. where we were told that she was in fact over medicated and they should have gotten her a doctor to evaluate her. On top of being over medicated, she also had bed sores that they were not tending to or trying to prevent. The director of nursing and the director of the facility did nothing about it.
