The facility is not a decent place. Right now , May, 28 2026 10:28 pm. They have to give me a lorazepam. It was not ordered for me. I’m trying to figure out what is going on. But it’s impassible to find out the truth. Will call tomorrow to Department of Health. By the way the doctor is working without license .

I cannot say enough wonderful things about this nursing center and rehabilitation facility. The staff makes you feel welcomed, supported, and truly cared for. The nurses and therapists are not only highly skilled and professional, but also compassionate and attentive to every detail of care. The rehabilitation team goes above and beyond to create personalized therapy plans that truly make a difference. The facility itself is clean, comfortable, and well-maintained, which gave us great peace of mind. Communication with family members is clear and consistent, and any questions or concerns are addressed promptly. Most importantly, the genuine kindness and respect shown to residents from the administrator, DNS, nurses, CNA, therapists etc. I would highly recommend this facility to anyone looking for exceptional nursing care and rehabilitation services. Thank you to the entire team for your dedication and hard work!

Overall I thought in all avenues and areas of care that the 3rd and 6th floor is phenomenal. Great nurses and aids and Phoebe the social worker was amazing. Her ability to follow up was beyond excellent and everyone was 10 steps ahead. It's a great thing when your loved one is in a facility that you don't have a worry or a concern. The maintenance crew were always on point. I'm so grateful because when she came in she was in a grave state and she was not at her baseline. The staff worked with her and she is now improved tremendously and is on the 6th floor. Kimberley from Link is phenomenal and makes sure everything is taken care of. The front desk is beyond exceptional, no matter what mood you're in when you walk in they make you feel better and ease your comfort. The therapy team has been sensational and dietitian has been great with reaching out and following up to assist. I had the same experience two years ago and feel the same again. Everyone is working in tandem which is very rare and bringing the best at all times. I wish I can give a 10 but can only give a 5.

On September 23,2024 I arrived at The Phoenix Rehabilitation and Nursing center with (2) Patellar Tendon Rupture Tear Surgeries to my knees along with A Fracture of the Right Tibia. I was at this facility for approximately 75 days. I left this facility on December 7,2024 During my stay at The Phoenix rehabilitation and nursing center i build a special bond with the staff members at this facility. The staff members at this facility worked hard to take care of me and get me back on my feet. I would like to give a special thank you to the following departments for the care that they gave to me & the facility. A) The physical therapy department gets a special thank you for getting me back on feet & never allowing me to give up B) The maintenance department gets a special thank you for always repairing issues that arise and for fixing my wheelchair C) A special thank you to the following CNA's that took such good care of me & easy my anxiety during my stay Ms.Lin heung Lui, Ms O'Reilly, Ms.Hodges Ms.Paul, Ms.Murrell, Ms.Hung, Ms.Standiford Ms. Zhilxin Li, Ms. Marie Brice, Ms.Conde Ms. St.Jude, Ms. Lin D) A special thank you to the amazing nurses for making sure i took my medications daily & keeping me positive Mr.Houston, Mr. Kyle Carroll, Ms.Carter, Ms.Zenida, Mr. Gregory, E) A special thank you to the housekeeping department for always keeping my room clean Ms. Beverly Johnson and ms. Tiffany F) A special thank you to the recreational department for all the activities G) A special thank you to the security department for keeping the facility safe. This facility is not without it's issues, but I can say my stay here wasn't bad at all. I actually made a few new friends. This facility has 24/7 visiting hours, food delivery is allowed. The food is not great here, some days are good, some days are bad. They have activities like bingo, church, movie night, etc Each patient has their own Televisions and their own phones to use.
