The worse rehab center ever. My grandpa was on the 2nd floor for 2 weeks where his carecwas horrible. Monique is the assistant DON. Horrible. She made promises on taking batter care of him. She failed twice! Then the administrator got involved. He failed us. They moved him to the 1st floor for better care. Suzanna is the DON. She failed. There was a agency nurse that removed his catheter by mistake! He was the wrong patient. Didn't check to make sure he was the correct patient. We took him out because lack of care. He died 1 week later at home. Hor

0 stars tbh but because I have to put one I will My granny who is 93 years old was in this facility an whe I say it only took a week and a half and she’s back in the hospital her blood pressure was DEATH LOW they were asked by my grandma to have her go to the hospital THEY IGNORED and finally had her go Saturday morning that’s not even the worse part! They don’t follow DR ORDERS THEY MAKE CHANGES ON THEY OWN. Dr told us her womb needed to be clean twice a day!! On the paper it showed 1 time a day. ( SO MY ADVICE ALWAYS KEEP YOUR COPY FROM YOUR DR) They said she had a heart problem and gave her heart medication which messed with her kidneys even more they decided to do a lab test on her ( we don’t even know why but found out her heart rate was to high and sodium level was low but just previously saying how Fine she was so when did that happen ??? They let my granny sit with a low blood pressure for 3 days before calling 9-1-1 One nurse even told my granny to use the bathroom on herself and she’ll come back and clean it ( starting a habit that she didn’t need) but when family was there they would come and pick her up and take her to the bathroom. They made up this thing of her not being able to swallow so they blended her food up NOBODY REQUESTED THAT but it’s ironic how she was able to swallow her pills just fine!! But not food? Doesn’t make sense and now where she is she’s eating food just fine isn’t that amazing ! There was nothing wrong with her They never really checked on her they unplugged her phone and placed it on the floor as well my granny couldn’t even reach the phone to touch so that wasn’t her doing nor did it look like a accident where she might of did it. They opened her window up knowing she gets cold fast soooo why touch her window ? Even greater my grandma had to give her a shower after being there from Sunday until May 1st she only had one shower and that’s because my grandma did it not staff. They have the audacity to be upset when u question them. And then they don’t fully follow instructions they were supposed to check blood pressure , and oxygen levels they never did the oxygen. They weren’t concerned for her to eat or nothing knowing she needed to be fed. So if family wasn’t there she was basically SOL. They do not check on residents like they should. The DON was the only person that actually took a action immediately after I made a complaint but everybody else especially one of the CNAs she wasn’t a cna but she she was given meds she’ll make u want to put hands on her cause she’s very snarky. DO. NOT SEND YOUR FAMILY MEMBER HERE IF U WANT THEM TO BE CARED FOR PROPERLY. They left my granny in bloody sheets that we have pictures of. They don’t care about the people in there nice clean facility on the outside but my grandmother room had shit on the toliet seat for a week and it smells like shit in there so who cleaning ? FLO she acts like she cares but she really doesn’t cause her up in a lie twice. My granny never saw a kidney Dr while she was there , the Dr never came the first week and the sad part is this place my my granny health WORSE instead of helping they never have to worry about her coming back this place needs to be shut down!!! If u don’t take nothing from this just know they don’t follow Dr orders and they will put your family members on what THEY WANT TOO. and claim they given them right meds. We kept telling them this isn’t right for her and nobody cared. I pray nobody has to go through this with they family member. Cause they show u one thing and when u leave your family member is not safe. Did I mention they also don’t feed them. My granny had physical therapy at 7:30 they skipped her breakfast nobody thought to come back and check on her and see if she ate I came around 11 she was hungry bc they never left her food. That happens twice. And then lunch is at 12 they didn’t feed her until around 1 knowing she’s supposed to be fed one time just came in and left the food don’t place in reach for her to get it smh this place is horrible!!!!

What an extreme disappointment of a place. Never put your family members in the facility! They come off very clean and a good facility like they will take good care of your family. That’s their rehab, but unfortunately, they do not care in their biggest motive as to get them in and out of there. My grandmother was there for not even two weeks and they were trying to push her out saying she was strong enough to come home we have been with Of rehab facilities and this one by far is the worst we’ve ever been to they are liars they say things that do not add up for your loved one. There is no family clear plans, and you will barely see anybody on the floor taking care of your loved one they leave them in their room to eat and and barely check in my grandmother never even ate. She would just sit there and stare at her food because she needs help with things being cut up or soft diet. The food is absolutely disgusting. Every time I would come I would come to find her soiled once again. My grandmother had no pants on one time and I was able to see at that time that she had a rash starting all over her body from their facility. The next time I came, she had a bruise on her side of her eye. The next time I come now, she’s got a cut on the side of her arm, but no one can give you an answer of why what happened and when you ask them, they say they’re going to check notes, but nothing comes up. The next time I come, there was feces all over the bathroom and dirty rags just laying there. I really don’t even think she even had a shower maybe even once there big liars please do your family a favor and do not take them to this place to get better. My grandmother is not even prepared to be home and she’s not strong whatsoever. It’s very sad. She will probably end up in the hospital very soon even though the goal was to keep her out of the hospital with her going to a rehab facility after the hospital made this plan for her. I have plenty of pictures of the facility, but Put them on the Internet But I do have the For documentation and will be going up to higher people on this matter.

If you are considering this facility for a loved one, I strongly encourage careful consideration based on our experience. When my brother was discharged from the hospital, facilities were screened due to the seriousness of his injuries and the level of care required. This facility was informed of his condition and indicated they could meet his needs. Unfortunately, our experience did not reflect that. Upon admission, his prescribed nutritional formula was not available. His pain medication was also unavailable at that time, and we were told staff were waiting for medications to be stocked. This raised immediate concerns for our family. My brother has a known esophageal injury and a history of aspiration related to vomiting. During his stay, feeding issues occurred that resulted in repeated vomiting overnight. Requests for anti-nausea medication were not addressed until later, and staff stated a physician was not available overnight. There were also concerns with staff interactions. On one occasion, when my brother used the call button for assistance, the response was inappropriate and distressing. Wound care did not appear to follow the documented medical orders. Only superficial dressings were changed, and deeper wound care instructions were not followed. Two days later, he was rehospitalized with an infection. Hospital staff expressed concern that wound care had not been performed according to orders. Medication administration was inconsistent, with pain medications not given as scheduled. On another occasion, a staff member asked if we could wait for assistance due to a personal break. This experience occurred in November 2025 and continues to cause our family distress. Based on what we observed, we recommend others carefully evaluate all options before choosing this facility.

Please read the previous reviews. I interacted with one person that seemed to have a genuine heart of gold toward the patients, and his job was a critical one. I don’t think my brother would have made it had it not been for this particular person and the expertise of the same. Everything else about the facility, including Minor stuff like, paper towel to dry your hands because you want to wash before you touch your loved one, was failing. I’m not a psychic, but I agree with the others, in saying this place should and I believe WILL Be shut down. Prayer works and my 70+ year old brother got out in time.
