Leaving this review as requested by Claudia and Jenny after they assisted in locating my loved one, who somehow managed to “safely discharge himself” from Sorrento in 100-degree weather with an unsteady gait and a cane. Thankfully, he was found safe. Unfortunately, the same level of urgency shown after he went missing was nowhere to be found before he walked out the front door unnoticed. To be clear, no one realized he was missing for nearly two hours. A vulnerable patient was allowed to leave without proper screening, supervision, or notification to family. Calling that a “safe discharge” is certainly… creative. What makes this even more concerning is the complete lack of accountability afterward. Families understand accidents and unexpected situations can happen. What is harder to understand is the dismissive attitude, poor communication, and apparent need to shift blame onto the patient and family instead of addressing obvious failures in protocol. Frank, the DON, offered little concern and even suggested, “This isn’t a locked unit, maybe you should look into putting him in one.” Interesting perspective considering the family had to involve SAPD and VIA to help search for someone your facility apparently believed had been discharged appropriately. Also concerning were inconsistencies in documentation that appeared more focused on protecting the facility than accurately reflecting events. That is not only unprofessional, but deeply unethical. And if this truly was considered an appropriate discharge, the follow-up text stating, “He’s found, he’s safe,” raises even more questions. Hopefully this situation leads to stronger safety measures, improved communication, and leadership willing to take accountability before another family experiences the same thing.

There are a lot of hard workers in this building. The extra amenities are nice and help with comfort and feeling closer to home. The court yards offer a safe place for your loved one to get fresh air with some nice landscaping. The Administrator Britt is great! As well as the nursing team with Don Frank are exceptional.
