Lifestyle & Care Options in Thousand Oaks, CA

You’re asked to wear a lot of hats in life: your career hat, your parent hat, your friend hat and now, you might find yourself wearing another hat, one as caregiver for your parent.  As a caregiver, you often balance on fine lines, like being available to keep an eye on your father in case he takes another fall, but giving him the privacy that he wants in his own home. Showing compassion and respect to your mother with memory loss, but making sure she’s eating, even if she doesn’t want to.

Chances are, you’ve asked yourself things like:

“How much longer can my mother live at home?”
“What happens if dad takes a fall in between my daily visits?”
“My mother can’t get out and do the things she used to do. Is she lonely?”

We understand that these are things you worry about, and we want to help.

A move to Sage Mountain can be the solution to give your mother or father the help they need, along with the peace of mind you’ve been searching for.  At Sage Mountain, people are there to help 24 hours a day, so while your parents will still be able to make their own decisions and retain the dignity they deserve, they will not be alone.

The care team at Sage Mountain has a passion for helping seniors and their families transition through aging.  They can be the vigilant eyes and the helping hand so you can focus on having a quality relationship with your mother or father. So, if you think it may be time for your family to consider a move to senior living, we encourage you to browse our lifestyle and care options and find the fit that's best for your family.

In the Moment® Memory Support at Sage Mountain

Our In the Moment® program is thoughtfully designed as a team-approach to memory care delivery. We describe what we offer as Memory Support because we’re intentionally shifting away from outdated Dementia care practices and into a hospitality model.

We recognize that each person we are privileged to serve is both unique in their life history, as well as their Dementia journey.  We understand the importance of a scheduled rhythm to each day. However, we also encourage our team members to slow down and embrace the impromptu moments that happen as well.  We find it’s the spontaneous moments that have high impact for both the care giver and the care receiver. Living In The Moment is the greatest gift we can give to anyone we serve during their journey.

Thousand Oaks Respite Care

For a lot of caregivers, taking care of their mom and dad is a full-time job. The idea of going out of town for a weekend or an unexpected short-term change in care needs, like doctor’s orders after being discharged from the hospital, are serious stressors for caregivers who already feel worn thin. If you’re a caregiver who needs some extra help for a few weeks or days, respite care might be the perfect fit.

Respite care provides the peace of mind of on-site care staff, prepared meals, and enrichment opportunities, without the long-term commitment. To find out more about our respite care services, contact us today or use the button below to schedule a tour.

Sage Mountain wanted to hire me.i am licensed in Ca for caregiving. The staff on the phone was abusive and rude and refused the manager's name and gave me the wrong extension after hanging up on me 4 times rudely. I see their license is revoked through Social Services. This was Thousand Oaks. Be careful.

I've been trying to reach the Executive Director for over a month!! He doesn't respond to emails, doesn't respond to Teams meeting requests, doesn't attend meetings his front desk schedules with me. My mother is a resident in the facility and I am POA. I find this very unprofessional and disturbing.

The community is truly a hidden gem--very private, secure, and safe. It offers a relaxing, calm, and family-oriented environment. From the moment you walk in, it feels like resort-style living. The staff are incredibly kind and welcoming, and the community is very active, with engaging activities happening every single day. The residents are highly involved, and there are frequent family visits, which adds to the warm and vibrant atmosphere. It is also a pet-friendly community, making it feel even more like home. If you are tired of living in a busy area with a fast-paced lifestyle and are ready to settle down, relax, and truly enjoy every moment of life, Sage is the place to be. This is a community filled with happiness and peaceful living--there is truly nothing like it.

Both of my parents (early 90's) moved in to Sage Mountain in early 2025. They and I did not know what to expect since this was their first time living in a home they did not own. Due to the assistance they now required, Sage Mountain was a perfect fit. They now can socialize and participate in activities they were unable to do on their own. The staff is extremely friendly and supportive. They love the food. It seems whenever I come in (at least once a week if not more) there is always a new activity going on related to the time of year. Highly recommend.
