I unfortunately would never recommend this place to anyone. My grandma has had so many problems here. It got so bad we had to move her somewhere else. From employee's unplugging her phone to charge their own, going through her drawers to find food without asking and mind you we always made sure she had snacks to share with everyone. To having an employee put her to bed at 5:30PM without dinner and when grandma told the CNA she didn't have dinner that CNA told her she didn't care. She has had 2 horrible falls 1 of them she hit her head and was laying in her blood for about an hour yelling for help. The other fall she fell out of the sit to stand lift, we later found out there was only 1 staff member operating the lift when there was supposed to be 2. Residents wait a minimum of 45 to 2+ hours to go to the bathroom. My Grandma's clothes were always missing and we had a sign on her closet that the family does the laundry all they had to do was put her dirty clothes in a bag we provided for them and they couldn't even manage that. There were times we would get other residents clothes. This place is so under staffed it isn't funny. Trying to get ahold of anyone from social worker, recreation, administration, etc is a joke. I can't tell you how many times administration would have a nurse call us instead of them or our calls and emails would never be returned. The food I'm not sure is even eatable, I wouldn't even feed it to my dog. They said they would have Grandma in physical therapy and there wasn't any physical therapy being done, in fact I don't even think they had a physical therapist coming in. The smell of the home reminds me of a porta potty you get hit with that smell the second you walk through the door. The amount of staff that actually cares is severely out numbered by the ones that do not care at all. So many times my grandma has called us up crying because they wouldn't listen to what she needed help with. If you love your family member, or not, I would highly recommend looking somewhere else. If I had to justify the 1 star it would be because of Nurse Bessie, Nurse Kim, hair dresser Cynthia, Nurse Diana and the few CNA's (I'm sorry I don't remember your names) that would go out of their way to take care of grandma. I could sit here for days and explain to you why not to send your loved ones here.

THIS PLACE NEEDS TO BE CLOSED DOWN AND INVESTIGATED! The amount of neglect they show towards these patients is insane. They wont feed them, wont change their diapers, wont check on your patient, and wont even care about your patient. One day i came into my granny room and she was faced towards the wall in the dark by herself like someone put her there (As seen in photo). Its full of messy employees who dont want to do their job like They couldn’t even write the doctor’s names on the board everyday like their SUPPOSED TO. My grandmother was a patient who just sadly passed due this rehab center not feeding her while in her care. Moreover, Im very upset at the fact that we trusted these people and they did her so wrong. They also have rats and when asked about it the worker said “probably yea”. PLEASE GET YOUR FAMILY OUT OF HERE ! THEYRE NOT BEING TAKEN CARE OF.

I’m pleased to say that all my concerns were addressed promptly. I have a friend in the facility that spoke highly about staff. In the visits that I participated in, my friend was taken care of and treated with dignity and professionalism, with exceptional privacy.The reason I give 4 stars is because water filters were due for a change. Ice machine was not working and the check-in kiosk needed to be sanitized when I checked in. I know with the diligent higher personnel, housekeeping will address and I’ll be back to give 5 stars. Thank you for following up and maintaining HIPPA standards and keeping my friend safe.

I can honestly say that I was treated like royalty at Palos Heights Rehab. Everyone came to introduce themselves to me and what their job was. My light was answered promptly. The food was good. It was so many people that went above and beyond their job duty. Tiff, Ryan, Joy, Joe, Jade, Rose, Tasha, Kalyssa, Angela, Deanna, Crystal, and those are just the names I can remember. There was no one who didn't do their job. I would definitely refer that rehab center. They also let family decorate your room so your more comfy with your stay. Oh yeah visiting hours are 24 hrs. You never have to be alone.

TLDR: This facility neglects their patients; their staff is aware and unchanging. Do NOT utilize Palos Heights Rehab Center. We had a terrible experience at Palos Heights Rehab. I would never send a loved one to this facility and I will be reporting them to the state. My grandmother was there for barely two days and it was an awful experience. There is no one monitoring the doors in the evening, the staff are unapproachable, unknowledgeable, and rude. The first night I came to see my grandma I had to search for an employee to direct me to her room. The place is filthy, it stinks, and the furniture in the rooms are used and beat. When my grandma arrived there were dirty linens in her room that did not belong to her. She was also served a pitcher of lemonade; I could scrape the old food remnants right off. The food didn't look, smell or taste like anything healthy nor edible. My grandma was left unattended overnight, not one person came to check in on her. She dropped her call button and had to yell for help, which no one answered. Seeing as she is immobile that is a MAJOR issue and safety hazard. She was given her medications late, and more than once did it take at least/over 45 minutes for a nurse/CNA to come to the room when the nurse call button was utilized. I spoke with Tiffany once and she had an extreme attitude. Kim Carr is not to be trusted. When I received copies of my grandma's medical records our phone call was documented. Kim has selective hearing because she did not include my complaints about the facility. She is fake to your face and isn't open to feedback. Her actions display neglect towards this nursing home/rehab center which in turn results in the neglect of the residents and patients in this facilities care. Shame on Kim for not advocating for the patients at Palos Rehab Center and for lying on patient records. There are a couple gems in this facility, and to be honest I feel bad they have such disappointing coworkers. In my experience their night shift is worse than their day shift. This facility is garbage and I hope all the people receiving care here are getting what they need, but I doubt it.
