Very unprofessional, Rude to family members. Don't change diapers often forget about patients all the time it's ridiculous,PLEASE DON'T LEAVE LOVED ONES HER THERE IS TOO MUCH ABUSE AND NEGLECT FROM RUDE STAFF THAT HARRASSES FAMILY MEMBERS EVEN AFTER THE PATIENT IS NO LONGER THERE AND THEY PUT A BLIND EYE THEY STILL HAVE THEM ALL WORKING THERE,AIDS ARE EVEN PULLING HAIR ON CLIENTS NOTHING GETS DONE ! FORGET TO FEED PATIENT DID FORGET 3 TIMES TO FEED MOM LEFT HER WET DIRTY DIAPER FOR 7-9 HRS NEVER ANSWER PHONE ALWAYS HAVE EXCUSES THEIR FOOD IS DISCUSTING PLEASE DON'T DO IT THEY ARE ALWAYS GETTING STATE COMING DOWN EVERYTHING IS DIRTY SMELL OF URINE AND FECES FORGET ABOUT PATIENTS FOR HRS WHILE THEY ARE ALL ON PHONES WALKING AROUND GOSSIPING NOT WORKING HAVE TOO MANY CITATIONS TO COUNT THIS PLACE NEEDS TO BE CLOSED!!!!

Run if Angelica Salas is still working there she don’t know what she is doing never worked as a DON ,she leaves pts in the dinning room unsupervised while eating she will not feed the feeders if she wants to get off early and not take the time to feed the slow eaters, she allows pts to have extension cords in their rooms which is a code violation,,she allows the units to be without a nurse because she allowed a staff nurse to come in at 9am the shift was 6to 6 therefore there was no nurse to worked the units, she would get mad at the other nurse because she would not take the keys I guess she still there cause corporate don’t want to take the time to hire a competent nurse ,because she is very incompetent and they know this ,she sits in her office all day fixing it up instead of helping with trays in the dinning room, taking pts that are dangerous And cause harm to other pts she is informed on staff not giving showers and she does nothing , she worked the units and refused to do the pts wound care ,

When I first got here I had a bad experience with one of the nurses but that was before the change in management. Since the change in management, I’ve had nothing but good experiences with CNA and nurses. The therapy team was my favorite thing, they’re very charismatic but stay focused on the job too

Been here since August. The food has improved since then,Thank heavens. The cnas and nurses are very kind,helpful and always willing to help. Remember if you treat everyone with respect and kindness that's how you will be treated. I have seen some of these patience be really rude and ugly to the cnas. CNAs just smile, do their job walk away. Some patience throw stuff at them as well. Yet the CNAs still smile and carry on their duty to help . Have there been a few that aren't so great? Yes. It happens. The Physical Therapy Zack is amazing . He definitely pushed me to do my best . I thank him as well Yet their not here anymore.. overall this really is an OK place

The staff at Avir (Northgate) did an exceptional job. We were so pleased with the care our loved one received while here. The rehab staff did a great job working with my daughter, and she made great progress during her stay. Thanks to Zach & Chris for all they did while working to get her mobile. The nurses showed so much compassion. Thank you.
