My aunt was sent home after their physical therapy report that stayed she was good to go. Of course that was after they strapped her to 2 people and dragged her around their little room a couple of times. Spoiler alert: She was NOT! She came home and was never able to get up again. She passed away 3 weeks later. After talking to other family members, one of whom is a nurse, I was told this is a common occurrence. If a patient is not going to get better they get them out as fast as possible so they do not die in their facility which lowers their rating. They actually DO have hospice options but they will never utter the word hospice and are not allowed to bring it up. I even asked outright if they were just sending her home to die and they said nothing. I was also on 2 occasions witness to extreme unprofessional behavior. One bordered on patient abuse and the other was so bad that a supervising nurse had to step in and handle the nurse. By the supervisor's admission, the offending nurse did not know she was on the premises and therefore felt emboldened to behave like that. I will do everything in my power to make sure no family or friends choose this facility if God forbid, one is ever needed.

I can’t imagine having a better experience anywhere. Our dad fell and had to be in rehab here for 60 days. We felt comfortable with everyone there. We knew he was loved when we’d come visit a had to leave him for the evening. We never worried once. They went above and beyond! We are grateful to everyone from the incredible front office, amazing aids, caring nurses, all the way to the lunch room “friends” and laundry ladies. We brought him home 2 days ago and he already misses them and we will be going to visit occasionally. It’s special there and they do their best to make everyone feel cared for.

My Grandpa was a patient back in 2011. I still can’t forget trying to get someone to help him use the bathroom. Like it bothers me so much to this day what he had to go through. After repeatly trying to get someone to come when he called, to trying to find someone myself. Then finally finding two people. I asked one and she told the other lady to come with and she said “ do I really have to”. I can’t get this memory out of my head all these years later. I can’t say for certain if things have changed but I will never recommend this place.

This place inst helpful. my grandma mother was here and she it would take 30-45 minutes for someone to come help her to the bathroom. The staff lost her glasses and when I called about they said they would called me back and never did. The place is very outdated the only thing dated was the physical therapy room that it’s. Also the staff was giving her to many medication at the same time that it started to mess with her head. Mind you my grandma is the type of person to bounce back from anyone surgery. So this wasn’t normal and when asked about a specific medication side effect they just said oh I have no clue. Also the insurance company say it’s a good place because of the ranking but it’s not. Please do your research before sending anyone here

My mom was sent to the Portage, Indiana location for rehab following surgery. The place was clean but the care was inconsistent. There isn’t enough staff to help patients with basic care such as using the bathroom when can’t move without assistance. The bathrooms are not accessible for those with mobility issues. The staff did not help my mom with rehab and when family helped, they were told that staff needed to be present to help. My mom started to have changes in medical which caused very obvious changes in her mental status and the staff didn’t seem concerned. I would not reccomend sending a loved one here. I was assured by the hospital staff that this place had a 5-star rating from medicare but I think medicare needs to look beyond the paperwork.
