This facility is awful. CNAS & nurses cussing, walking through the halls & half the time not even in uniform so i wasn’t sure who was who and what their names were. . this isn’t the first time I’ve heard this type of talk. Weekend shifts are ridiculous. Staff act like they run the building you can’t even tell a difference when management is in the building as well, they still act ridiculous. My father had nothing good to say but the cleaning girls were was kind to him and the food wasn’t awful. I’m so upset and so glad he’s out of here. All i can say is the bad reviews are true and please listen and do your due diligence!

I took my mother here, and all i heard was cussing and nonsense. Staff was sitting at nurses station while call lights were going off, my mother was soaked half the time. Don’t even get me started with the weekend staff . They are awful and unprofessional.

The food at this facility is TERRIBLE! The elderly deserve much better than this. If I did not bring in food for mother, she would starve to death. I am sure lots of money is wasted because of the un-eaten food. The staff however is loving and caring.

Our first experience dealing with a nursing home environment. The staff at McCormicks Creek was amazing. From the initial phone calls to end of life for our loved one was exceptional. A big thank you to the nursing staff, Melissa and Sherri for helping us navigate this challenging time in our life’s. We now how new friends and we would highly recommend this facility.

Sharing our family’s experience so others can make informed decisions. My mother was transferred from Bloomington Hospital to McCormick’s Creek Rehabilitation Center following a fall, a broken hip, and emergency surgery. The transfer occurred overnight, and our family was not notified in advance. Upon arrival, she was left unattended without an effective way to call for assistance and used her personal phone to contact family at approximately 4:00 a.m. She reported being left unclothed in her bed. A family member contacted the nursing station and was told staff would go to her room. She was later placed in a diaper. During her stay, she was placed in a diaper despite requesting assistance to use the bathroom and was informed this was standard practice. A bedside commode was requested and denied. On more than one occasion, she waited several hours for assistance after activating the call button, which does not directly alert staff but instead activates a hallway light. She received approximately 10 minutes of physical therapy during her 72-hour stay. There were also inconsistencies in information provided to our family regarding physician visits, including conflicting dates and times. To our knowledge, she did not have a physician visit during her stay. Multiple staff members independently advised our family to report our concerns to the state and indicated they would not place a family member at this facility, stating their hands were tied. Today, we removed our mother from McCormick’s Creek and transported her home ourselves after being told medical transport would not be arranged because we were removing her against medical advice. As a family, we did not feel the environment was safe or appropriate for her recovery. We are currently contacting other local rehabilitation centers to ensure she receives appropriate care. We are in the process of filing reports with the appropriate state agencies, including Adult Protective Services, so these concerns can be reviewed. Our intent in sharing this is accountability and awareness. Families deserve transparency, dignity, and appropriate care when trusting a rehabilitation facility with their loved ones.
