Livewell Connecticut in Plantsville is an assisted living community with skilled nursing care that specializes in supporting people living with all forms of dementia, including Alzheimer's Disease. The community features residential assisted living apartments and skilled nursing services, with expansion planned for completion in 2025.
The community operates the Resilient Living Center, which offers a distinctive approach to dementia care through evidence-based cognitive wellness programs. In partnership with Yale School of Nursing, Livewell has launched a FINGER-based cognitive wellness clinic led by dementia prevention experts. This clinic applies proven multidomain lifestyle interventions—exercise, nutrition, cognitive training, and vascular health management—designed to slow cognitive decline. The center also provides one-on-one wellness consultations with individualized heart and brain health screenings.
Residents and visitors benefit from diverse programming and social engagement. The community hosts live concerts in The Gathering Place Cafe courtyard on the 1st and 3rd Thursday of each month from June through September. The Resilient Living Center offers a catalog of over 50 in-person and virtual programs including live performances, workshops, and discussion groups tailored to people at all cognitive levels. Resource Coordinators help individuals create personalized roadmaps using occupational therapy, personal navigators, and other support services to manage cognitive changes and maintain focus on meaningful connections and activities.
Educational offerings include monthly public sessions on cognitive resilience and risk reduction, quarterly dementia risk-reduction workshops for clinicians, and complimentary LiveWell 101 sessions—small group introductions available both virtually and in-person for anyone interested in learning about the community's resources and approach to living well with or preventing dementia.
