My grandmother recently needed to go here for rehabilitation. The clinical staff showed lots of support and did well on daily notes. However, if it is your first time going into some sort of placement whether its short or long term care. I recommend speaking to Business Managers and Social Work teams before hand to get answers you may need. I had a little bit of a rough time at first but worked over it all with the help or Mary Medina and Jessica. They both redirected me in better directions for my loved one and I got assistance with resources I needed.

Life Care Center of Farmington is truly a great place for the care of your loved one. I recently had to make the difficult decision to have my grandmother cared for by skilled professionals and this facility has helped to make that decision a lot easier. Every single person that I encountered during this process has been nothing short of kind, helpful and full of compassion. The office manager Mary made the process of admittance and the process of medicaid funding so easy and she was very knowledgeable about everything. I can't recommend this facility enough.

I’m giving Life Care Center of Farmington 1 star for improvement on their care. My grandfather was admitted there for rehabilitation, where the goal is supposed to be helping patients improve their strength, mobility, and independence after illness or hospitalization. Unfortunately, our family's experience was very disappointing. We felt that staff did not adequately attend to his needs. My grandfather developed a severe urinary tract infection (UTI), and we were concerned that his catheter was not being properly cleaned and maintained. Watching a loved one suffer from a preventable complication was heartbreaking. Families place their trust in rehabilitation and nursing facilities to provide attentive, compassionate care. In our experience, that standard was not met. I strongly encourage anyone with a loved one in a rehabilitation or nursing facility to visit often, ask questions, and make sure their family member is receiving the care they need. This was our personal experience, and I hope sharing it helps other families stay informed and involved in their loved ones' care.

My loved one was surprised to hear he was going to LifeCare in Farmington after his hospitalization. He went in with a positive attitude and felt terrific when he left. With therapy 2 hours every day it really helped and I could see the improvement as he got in and out of bed on his own!! Life care staff are the best around and we won’t go anywhere else!

My husband's health changed very suddenly and it was obvious that I could not give the care he needed at home. Mary Medina, the business manager was so helpful with the initial Medicare coverage benefits and then with helping us understand private pay when he was moved to the memory care unit. The staff at Life Care have made his transition as comfortable as possible which reassures me he is being well take care of. I am grateful to all them for making his experience the best it can be for where he is in his life now. The staff also helped me during this time of transition. Thank you to all of them.
