Basin Home Health & Hospice is a home health, hospice, and caregiver services provider headquartered in Farmington, New Mexico, serving families across Northwest New Mexico including San Juan County, Gallup, and Grants. The organization has been locally owned and operated since 1992 and is the largest locally-owned provider of home health and hospice services in San Juan County.
